Koroner arter bypass cerrahisinde uygulanan hafif hipotermi ve normoterminin serum parametrelerine olan etkisinin karşılaştırılması

Abstract

Günümüzde koroner arter bypass greft ameliyatı olan hastalarda özellikle ameliyat sonrası dönemde miyokardiyal disfonksiyon için farklı farklı önleyici yöntemler geliştirilmiştir . Bunlar arasında en sık kullanılan yöntemlerden birisi aortik kros klemp süresince hipotermi uygulamasıdır. Özel Çorlu Reyap Hastanesi Kalp Damar Cerrahisi kliniğinde elektif şartlarda hafif hipotermik ve normotermik kardiyopulmoner bypass altında, koroner bypass operasyonu yapılan toplam 30 hasta çalışmaya rızaları alınarak dahil edildiler. Koroner Arter Bypass Greftleme (KABG) operasyonu olan 30 hasta normotermik (n=15) ve hipotermik (n=15) olmak üzere 2 grupta retrospektif olarak incelemeye alınmıştır. Hastaların yarısına aortik kros klemp süresince, hafif hipotermi uygulandı. Hastaların diğer yarısına, aortik kros klemp süresince normotermi uygulandı. Kardiyak normoterminin postoperatif serum parametrelerine (lökosit, trombosit ve hemoglobin sayımı alınmış ve ACT bakılmıştır, biyokimya örneklerinden ise AST, ALT, Kreatinin, Üre, CRP değerleri incelemeye alınmıştır.), kan ürünü replasman miktarı (TDP ve ES) ve drenaj miktarına olan etkisi incelendi. Kliniğimizde izole primer koroner bypass cerrahisi uygulanan hastalardan, istatistiksel olarak anlamlı olmasada kardiyak hafif hipotermi grubunda, kardiyak normotermi grubuna göre inflamatuar yanıt daha azdı. Fakat gruplar arası serum parametrelerinde sonuç istatistiksel açıdan anlamlı değildi (p>0.05). Üre değeri için; p=0.506, AST değeri için; p=0.848, ALT değeri için; p=0.749, kreatinin değeri için p=0.374, CRP değeri için p=0.585, WBC değeri için; p=0.844, Hgb değeri için; p=0.915, Trombosit değeri için; p=0.322, ACT değeri için p=0.595 değerleri bulunmuştur. Aynı zamanda hafif hipotermi grubunda istatiksel açıdan anlamlı olarak normotermi grubuna göre daha az drenaj tespit edildi (p<0.05). Drenaj miktarı için p=0.003 değeri bulunmuştur. Kan ürünü replasman miktarı açısından ise sonuç anlamlı bulunmamıştır (p>0.05). TDP kullanımı açısından p=0.579 ve ES kullanımı açısından p=0.127değerleri bulunmuştur. Sonuç olarak elektif şartlarda, normotermik kardiyopulmoner bypass ile hafif hipotermik kardiyopulmoner bypass arasında postoperatif serum parametreleri ve kan ürünü replasman miktarı açısından çok büyük bir fark olmamak ile birlikte drenaj miktarı açısından hafif hipotermi daha güvenlidir.Currently, different preventive measures have been developed for patients with coronary artery bypass graft surgery, especially for postoperative myocardial dysfunction. Among these, one of the most commonly used methods is hypothermia during aortic cross clamping. A total of 30 patients undergoing elective coronary artery bypass grafting under mild hypothermic and normothermic cardiopulmonary bypass were included in the study at the Special Corridor Hospital Cardiovascular Surgery clinic. Thirty patients with coronary artery bypass grafting (CABG) operation were retrospectively reviewed in two groups as normothermic (n = 15) and hypothermic (n = 15). Half of the patients underwent mild hypothermia during aortic cross clamping. The other half of the patients underwent normothermia during aortic cross clamping. The effects of cardiac normothermin on blood product replacement (TDP and ES) and the amount of drainage were investigated in terms of postoperative serum parameters (leukocyte, platelet and hemoglobin counts were taken and ACT was taken and biochemical samples AST, ALT, creatinine, urea and CRP values were taken into account) . In our clinic, patients with isolated primary coronary artery bypass surgery had statistically significant lower cardiac hypothermia group than group with cardiac normothermia. However, the results were not statistically significant in serum parameters between groups (p> 0.05). At the same time, in the group of mild hypothermia, statistically significant less drainage was detected than in the normothermic group (p <0.05). The drainage amount was found to be p = 0.003. As a result, there is no significant difference between normothermic cardiopulmonary bypass and mild hypothermic cardiopulmonary bypass in terms of postoperative serum parameters and blood product replacement in elective conditions, while mild hypothermia in terms of drainage amount is more reliable.
