Subakromial sıkışma sendromu tedavisinde iyontoforez ve fonoforez uygulamasının ağrı, fonksiyonel düzey ve proprioseptif duyu üzerine etkisi

Abstract

Çalışmamızın amacı subakromial sıkışma sendromlu hastaların tedavisinde fizyoterapi programına ek olarak yapılan iyontoforez ve fonoforez uygulamalarının ağrı, özürlülük, fonksiyonel düzey ve propriosepsiyon üzerine etkilerini araştırmaktır. Çalışmamızda subakromial impingement tanısı almış 18-65 yaş arasındaki 43 hasta randomize olarak iki gruba ayrıldı. Birinci gruba (n=22) diklofenak sodyum iyontoforezi, ikinci gruba (n=21) diklofenak sodyum fonoforezi ve her iki gruba da yüzeyel sıcak, transkutanöz elektriksel sinir stimülasyonu (TENS) ve egzersizden oluşan fizyoterapi programı uygulandı. Gruplar 10 seanslık tedavi programı öncesi ve sonrası ağrı, omuza özel testler, eklem hareket açıklığı, kas kuvveti, omuz özürlülük skorlamaları ve propriosepsiyon açısından değerlendirildi. Çalışmamız sonucunda her iki grubun omuz eklem hareket açıklığının arttığı, ağrı, fonksiyonel düzey ve özürlülük skorlarında anlamlı düzelmeler olduğu belirlendi (p<0,05); fakat gruplar arasında istatistiksel açıdan fark saptanmadı (p>0,05). İyontoforez grubunda internal rotasyon dışındaki kas gücü değerlerinin, fonoforez grubunda ise tüm kas gücü değerlerinin anlamlı ölçüde arttığı görüldü (p<0,05). Propriosepsiyon ölçümlerinde iyontoforez grubunda fleksiyon, internal ve eksternal rotasyon hareketlerinin açısal sapmalarında (p<0,05) , fonoforez grubunda ise fleksiyon ve ekstansiyon açısal sapmalarında (p<0,05) belirgin iyileşme saptandı; iki grup arasındaki fark anlamlı bulunmadı (p>0,05). Bu çalışma impingement sendromlu hastalarda uygulanan diklofenak sodyum iyontoforezi ve fonoforezinin omuz fonksiyonları üzerine benzer pozitif iyileştirici etkilere sahip olduğunu göstermiştir.The aim of the study was to investigate the effects of applications of iontophoresis and phonophoresis, which has been used in addition to physiotherapy program for the treatment of patients with subacromial impingement syndrome, on pain, disability, functional level and proprioception. In the study, 43 patients between 18 and 65 years of age who diagnosed with subacromial impingement were divided into two groups randomly. Diclofenac sodium iontophoresis was applied to the first group (n=22), diclofenac sodium phonophoresis was applied to the second group (n=21), and the physiotherapy program including superficial heat, transcutaneous electrical nerve stimulation (TENS) and exercise was applied in the both groups. The groups were assessed with regard to pain, shoulder specific tests, range of motion, muscle strength, shoulder disability scorings and proprioception before and after the treatment program with ten sessions. The results of the study show that range of motion of shoulder was increased, there are significant improvements in pain, functional level and the disability scores in the both groups (p<0,05), however statistical significance was not determined between groups (p>0,05). In the group of iontophoresis, the values of muscle strength except internal rotation, and in the group of phonophoresis, all values of muscle strength were significantly increased (p<0,05). Significant improvements were observed in the group of iontophoresis (p<0,05) regarding to angular deviation of flexion, internal, external rotations, and in the group of phonophoresis (p<0,05), regarding to angular deviation of flexion, extansion at the proprioceptive measurements, and the difference between groups was not significant (p>0,05). The study shows that diclofenac sodium iontophoresis and phonophoresis that are used in patients with subacromial impingement syndrome have similar positive therapeutic effects on shoulder functions.
