İntrauterin mort fetüs ve fetal anomali nedeniyle tıbbi terminasyon yapılan hastaların tahliye süreçlerinin karşılaştırılması

Abstract

Amaç: İntrauterin mort fetüs endikasyonu ile tıbbi tahliye yapılan hastaların fetal anomali endikasyonu ile tıbbi tahliye yapılan hastalar ile obstetrik veriler ve tahliye sürecine dair veriler bakımından karşılaştırılması planlanmıştır. Bu olgular için doğru yaklaşım geliştirebilmek ve yönetimini daha iyi yapabilmek amaçlanmıştır. Gereç ve yöntem: Medipol Mega Üniversite Hastanesi Kadın Hastalıkları ve Doğum kliniğinde 10 hafta ve üzeri gebelik terminasyonu yapılmış hastaların retrospektif olarak taranması yoluyla; intrauterin mort fetüs endikasyonu ile tıbbi tahliye işlemine alınmış olan hastalarla ilişkili klinik parametrelerde fetal anomali nedeniyle tıbbi tahliye yapılmış hastalara kıyasla farklılıklar olup olmadığı ortaya konmuştur. Çalışma yapılırken, Medipol Mega Üniversite Hastanesi'ndeki tüm Kadın Hastalıkları ve Doğum hekimlerinin ortak veri girişi yaptığı Pusula sisteminden faydalanılarak veriler taranmış ve dökümante edilmiştir. Hastaların obstetrik anamnezlerinde yer alan gravida, parite, abortus, ektopik gebelik, önceki gebeliklerindeki doğum şekli verileri ile kaçıncı haftada tıbbi tahliye yapılması gerektiği, tahliye ve yatış süreleri, uygulanan tıbbi yöntemler ve gelişen komplikasyonları içeren veriler tespit edilerek tahliye süreçleri karşılaştırılmıştır. Bulgular: Anomalisi olan grubun gravida, parite, abortus ortalaması daha az izlendi. Bu grupta gebelik haftası belirgin olarak daha ileriydi. Anomalisi olan hastaların tahliye süresinin daha uzun sürdüğü görüldü; ancak yatış süreleri daha kısaydı. Daha önce doğum yapmış olan hastalarda, doğurmayanlara kıyasla tahliye süresinin daha kısa olduğu görüldü. Bunun yanında, anomalili gruptaki hastalarımızın anomali olmayanlarla karşılaştırılmasında; önceki doğum şekilleri bakımından, normal vajinal yolla doğumun belirgin olarak daha az olduğu saptandı. Misoprostol kullanımı açısından fark izlenmezken, balon kullanımına anomalili grupta çok daha fazla başvurduğumuzu gördük. Histerotomiye ise anomali olmayan grupta belirgin olarak daha az başvurmuştuk. Anomalisi olan grupta küretaj işlemi daha çok uygulanmıştı. Anomalili olan grupta, fetosit prosedürü uygulanan olgular mevcuttu. Komplikasyon 173 vakadan 4'ünde karşımıza çıktığından, gruplar arası fark saptanmadı. Sonuç: Endikasyon farklılıklarına göre tıbbi tahliye prosedürüne alınan hastaların yönetimleri değişkenlik gösterebilmektedir. Obstetrik anamnezlerindeki farklılıklar, tahliye sürelerini etkilemektedir. Terminasyonda başvurulan yöntemlerin de bu doğrultuda değişmesi dikkat çekicidir.Objective: It is planned to compare the patients who underwent medical evacuation with the indication of intrauterine mort fetus with the patients who underwent medical evacuation with the indication of fetal anomaly, in terms of obstetric data and data on the evacuation process. It is aimed to develop the right approach for these cases and to make their management better. Material and method: Through retrospective screening of patients who had a pregnancy termination of 10 weeks or more in the Medipol Mega University Hospital Gynecology and Obstetrics clinic; it has been demonstrated whether there are differences in clinical parameters related to patients who were taken to medical evacuation with the indication of intrauterine mort fetus compared to patients who were medically evacuated due to fetal anomaly. During the study, the data were scanned and documented by using the "Pusula" system, where all gynecologist and obstetricians in Medipol Mega University Hospital entered common data, and the data were scanned and documented. The data including the need for evacuation, the duration of evacuation and hospitalization, the medical methods applied and the complications that developed were determined and the evacuation processes were compared. Results: In the group with anomaly, the mean of gravida, parity and abortion was less. In this group, the gestational week was significantly more advanced. It was observed that patients with anomalies took longer to evacuate; however, the length of stay was shorter. It was observed that the evacuation period was shorter in patients who had given birth before, compared to those who did not. In addition, when comparing our patients in the anomaly group with those without anomalies; in terms of previous delivery methods, it was determined that the normal vaginal delivery was significantly less. While no difference was observed in terms of misoprostol use, we found that we used balloons much more frequently in the anomaly group. We applied to hysterotomy significantly less frequently in the non-anomaly group. Curettage was used more frequently in the group with anomaly. In the group with anomalies, there were cases in which the fetocyte procedure was applied. Since the complication was encountered in 4 out of 173 cases, no difference was found between the groups. Conclusion: The management of the patients who are taken to the medical evacuation procedure may vary according to the differences in indications. Differences in obstetric anamnesis affect evacuation times. It is noteworthy that the methods used in termination have also changed in this direction.
