Açık kalp cerrahisinde pompa süresi uzayan vakalarda entegreli ve entegresiz oksijenatör kullanımına bağlı preop ve postop dönemlerdeki böbrek fonksiyon değerleri

Abstract

Perfüzyon; bir taşınma ortamıyla kan ve/veya benzer bir sıvının hücre, doku ve organların ihtiyaç duyduğu metabolik yapı taşlarının taşınması durumudur. Kardiyo Pulmoner By-Pass (KPB) cerrahisinde kalbin pompa fonksiyonunun ve akciğerin gaz değişim fonksiyonunun geçici olarak hastanın damar sistemine bağlanan bir pompa ve bir oksijenatör (Kalp Akciğer Makinesi) ile sağlandığı sisteme perfüzatör denir. Oksijenatörler ekstrakorporeal dolaşım sırasında hastanın akciğerlerinin görevlerini üslenen aygıtlar olarak tanımlanırlar. Ancak adlarının gösterdiği gibi yalnız oksijen transportu değil, karbondioksit, anestetik ajanlar ve başka gazların da dolaşımdan alınması ve verilmesini sağlarlar. Ekstrakorporeal sirkülasyon parçaları içinde kanın temas ettiği en geniş yabancı yüzeyi oluşturmaktadır bu nedenle de kan elemanlarının en büyük hasara uğradığı bölümdür. Bu çalışmanın amacı; uzamış erişkin açık kalp ameliyatı sırasında kullanılan entegreli ve entegresiz oksijenatörlerin preop ve postop üre-kreatinin düzeylerini karşılaştırmaktır. Araştırmaya İstanbul Medipol Üniversitesi Hastanesinde 01.12.2019-28.02.2020 tarihleri arasında açık kalp ameliyatı olan 60 (30 entegreli oksijenatör kullanılan, 30 entegresiz oksijenatör kullanılan) hasta örneklem olarak alınmıştır. Veriler SPSS 22.0 for Windows programı ile bilgisayar ortamında analiz edildi. Öncelikle verilerin normallik dağılımı için Kolmogorov-Smirnov testi yapıldı ve normal dağılmadıkları tespit edildi. Bu nedenle parametrik olmayan testlerden Mann Whitney U ve Wilcoxon işaret testleri uygulandı. Tanımlayıcı bilgiler için sayı ve yüzde analizi; üre ve Kreatinin için ortalama ve standart sapma analizi yapıldı. İstatistiksel anlamlılık p<0,05 olarak kabul edildi. Çalışma sonucunda; entegresiz oksijenatör kullanılan hastaların ameliyat öncesi Kreatinin değerlerinin ameliyat sonrası istatistiksel olarak düştüğü saptanmıştır. Fakat entegre ve entegresiz oksijenatörlerin ameliyat sonrası üre-kreatinin değerlerinin karşılaştırılmasında anlamlı bir farklılığa rastlanmamıştır.Perfusion; It is the transport of metabolic building blocks of blood and / or a similar fluid with a transport medium required by cells, tissues and organs. Oxygenators are defined as devices that take on the functions of the patient's lungs during extracorporeal circulation. However, as their names indicate, they provide not only oxygen transport, but also carbon dioxide, anesthetic agents and other gases from the circulation. It forms the largest foreign surface in contact with the blood in the extracorporeal circulation parts, therefore it is the part where the blood elements suffer the greatest damage. The aim of this study is; to compare the preoperation and postoperation urea-creatinine levels of integrated and non-integrated oxygenators used during adult open-heart surgery. 60 patients (30 integrated oxygenators and 30 non-integrated oxygenators) who underwent open heart surgery between 01.12.2019-28.02.2020 at Istanbul Medipol University Hospital were included in the study. The data were analyzed in computer environment with SPSS 22.0 for Windows program. First of all, Kolmogorov-Smirnov test was performed for normality distribution of the data and it was found that they were not distributed normally. Therefore, Mann Whitney U and Wilcoxon sign tests, which are nonparametric tests, were used. Number and percentage analysis for identifying information; Mean and standard deviation analyzes were performed for urea and creatinine. Statistical significance was accepted as p <0.05. In the results of working; It was determined that preoperative creatinine values of patients who used non-integrated oxygenator statistically decreased postoperatively. However, no significant difference was found in the comparison of postoperative urea-creatinine values of integrated and non-integrated oxygenators.
