Multiple skleroz hastalarında grup telerehabilitasyonu

Abstract

Genç erişkinlerde görülen, santral sinir sistemini etkileyen ve otoimmün inflamatuar demiyelinizasyona yol açan Multiple Skleroz (MS), dünyada travmaya bağlı olmayan özürlülük sebepleri arasındadır. Bu araştırmanın amacı EDDS (Genişletilmiş Özürlülük Durumu Ölçeği) puanına göre iki gruba ayrılmış olan Multiple Skleroz hastalarına grup tabanlı bilişsel egzersiz tedavisinin etkinliğinin araştırılmasıdır. Çalışmada 6 haftayı kapsayacak şekilde, haftada 2 kez 40'ar dakika uzaktan bağlanarak grup rehabilitasyonu yapılmış olup diğer günlerde ev ödevi; kişiye özel etkilenen bilişsel alanlara göre telefon uygulamaları ile egzersizlere ve hastanın yapamadığı aktiviteler üzerine yoğunlaşmaları istendi. 6 hafta sonunda tekrar hastalar bilişsel açıdan değerlendirildi. Yapılan değerlendirmede Montreal Bilişsel Değerlendirme Testi (MOBID), Sayı Menzili Testi, Adımlı İşitsel Seri Ekleme Testi (PASAT), Sembol Sayı Modaliteleri Testi (SDMT), Sözel Akıcılık Testleri (Meyve İsim, K-A-S), İz Sürme Testi, Sözel Bellek Süreçleri Testi (SBST), Saat Çizimi, Barthel İndeksi, Beck Depresyon Ölçeği, Fonksiyonel Ambulasyon Sınıflaması (FAS), Kanada Aktivite Performans Ölçümü (KAPÖ) kullanıldı. Rehabilitasyon sonrası nöropsikolojik testler sonucunda hastalarda genel bilişsel durum, dikkat, bellek, yürütücü işlevler, görsel mekânsal işlevler ve bilgi işlemleme hızı anlamlı bir şekilde iyileşme göstermiştir. Yapılan diğer test sonuçlarında ise rehabilitasyon gruplarında hastaların depresyon ve ortalama memnuniyet skorlarında anlamlı bir iyileşme gözlenirken; EDSS skoru 5.5 ve üstünde olan gruptaki hastalarda ortalama memnuniyet skorlarında da anlamlı iyileşme gözlenmiştir. Telerehabilitasyonun, MS hastalarında bilişsel rehabilitasyon tedavisi için kullanılabilecek etkin ve kolay uygulanabilir farmakoloijk olmayan yöntem olduğu düşünülmüştür.Multiple Sclerosis (MS), which is seen in young adults, affects the central nervous system and causes autoimmune inflammatory demyelination, is among the non traumatic causes of disability in the world. The aim of this study is to investigate the effectiveness of group-based cognitive exercise therapy in Multiple Sclerosis patients, who were divided into two groups according to their EDDS (Extended Disability Status Scale) score. In the study, group rehabilitation was done by connecting remotely for 40 minutes twice a week, covering 6 weeks, and homework on other days; They were asked to focus on phone applications and exercises and activities that the patient could not do, according to the cognitive areas that were affected by the individual. At the end of 6 weeks, the patients were evaluated cognitively again. In the evaluation, Montreal Cognitive Assessment Test (MOCA), Digit Span Test, Paced Auditory Serial Addition Test (PASAT), Symbol Digit Modalities Test (SDMT), Verbal Fluency Tests (Fruit-Name, K-A-S), Trail Making Test, Verbal Memory Processes Test, Clock Drawing, Barthel Index, Beck Depression Inventory, Functional Ambulation Classification, Canadian Occupational Performance Measure (COPM) were used. As a result of neuropsychological tests after rehabilitation, general cognitive status, attention, memory, executive functions, visuospatial functions and information processing speed improved significantly. In other test results, a significant improvement was observed in the depression and average satisfaction scores of the patients in the rehabilitation groups; A significant improvement was also observed in the mean satisfaction scores of the patients in the group with an EDSS score of 5.5 and above. Telerehabilitation is thought to be an effective and easily applicable non pharmacological method that can be used for cognitive rehabilitation treatment in MS patients.
