MR-linak cihazında yapılan tedavi planlarının arccheck® kalite kontrol sistemi ve iyon odası ile değerlendirilmesi

Abstract

MR-Linak, manyetik rezonans görüntüleme tekniği ve radyoterapi cihazının birleşimi ile oluşan bir tedavi cihazıdır. Bu tedavi cihazında şimdilik yoğunluk ayarlı radyoterapi tekniği uygulanabilmektedir. Hastaların günlük kritik organ ve hedef hacimlerine yönelik tekrardan konturlama ve planlama yapılmaktadır. Bu çalışmanın amacı MR-Linak cihazında yoğunluk ayarlı radyoterapi uygulanan hastaların tedavi planlarının ArcCHECK® kalite kontrol sistemi ve onun içerisine yerleştirilen iyon odası ile değerlendirilmesidir. Bu çalışmada kullanılan ArcCHECK® ve iyon odası tedavi planlama sistemine tanıtılmıştır. Uygulanan tedavi planları doz haritaları kalite kontrol sistemi bilgisayar programı vasıtası ile değerlendirilmiştir. İyon odası, ArcCHECK® kalite kontrol sistemi merkezine denk gelecek şekilde ayarlanmıştır. Tedavi planlama sisteminde, tedavi planlarının ArcCHECK® üzerinde ve tam iyon odasının referans noktasında oluşturduğu dozlar okunmuştur. Referans olması amacı ile 10x10cm boyutunda alan 100 monitör birim (MU) tedavi planlama sisteminde hesaplatılıp iyon odası referans noktasındaki doz okunmuştur. Tedavi cihazında bu referans alan ışınlanmıştır. Bu alanın oluşturduğu elektrometre yük değeri not alınmıştır. Referans alanın doz değerinin ve onun oluşturduğu elektrometre yük değeri kullanıldı. Tedavi planının merkezdeki iyon odası referans noktası doz değeri ve oradan da elektrometre yük okuması doğru orantı olacak şekilde beklendi ve hesaplandı. ArcCHECK® kalite kontrol sistemi değerlendirmesi ve iyon odası okuma değerlendirmesi, 10 hipofraksiyone prostat ve 5 sbrt prostat hastasında sırasıyla gama analiz th10-%3-2mm ve th10-%2-1mm kabul kriterleri içerisinde olduğu bulundu.MR-Linac is a treatment device consisting of a combination of magnetic resonance imaging technique and radiotherapy device. In this treatment device, intensity-modulated radiotherapy technique can be applied for now. Recontouring and planning is done for the daily critical organ and target volumes of the patients. The aim of this study is to evaluate the treatment plans of patients who underwent intensity modulated radiotherapy in the MR-Linac device, with the ArcCHECK® quality control system and the ion chamber placed in it. ArcCHECK® and ion chamber used in this study are introduced to the treatment planning system. The applied treatment plans were evaluated by means of dose maps, quality control system computer program. The ion chamber is set to coincide with the ArcCHECK® quality control system center. In the treatment planning system, the doses created by the treatment plans on ArcCHECK® and at the reference point of the ion chamber were read. As a reference, 100 monitor units (MU) in a 10x10 area were calculated in the treatment planning system and the dose at the reference point of the ion chamber was read. In the therapy device, this reference field is irradiated. The electrometer charge value created by this field was noted. The dose value of the reference field and the electrometer charge value formed by it are known. The dose value of the ion chamber reference point in the center of the treatment plan and the electrometer load reading from there were expected and calculated in direct proportion. ArcCHECK® quality control system evaluation and ion chamber reading evaluation were found to be within the acceptance criteria of gamma analysis th10-3-2%mm and th10-2-1mm in 10 hypofractionated prostate and 5 sbrt prostate patients, respectively.
