Sleeve gastrektomi hastalarının beslenme durumunun değerlendirilmesi: Prospektif randomize klinik çalışma

Abstract

Dünya çapında obezite, giderek artan sağlık problemi haline gelmektedir. Bariyatrik cerrahi, kalıcı kilo kaybının sürdürülmesini sağlamakta, bununla birlikte obeziteye bağlı komorbiditelerin ilerlemesini önemli derecede önlemektedir. Bariyatrik cerrahi yöntemlerinden biri olan Sleeve Gastrektomi (SG), son yıllarda güvenirliliği artan ve sık uygulanan bir yöntem haline gelmiştir. Bu nedenle bizim çalışmamızda postop 6 ayda, hastalara bariyatrik cerrahi kurallarına göre uygulanan beslenme programının, dönemler arasındaki etkisi araştırılmıştır. Çalışma Bursa Uludağ Üniversitesi Tıp Fakültesi Hastanesi'nde, Eylül 2016-Haziran 2017 tarihleri arasında yapılmıştır. Çalışmamıza 18-65 yaş grubu arasında, SG ameliyatı geçirmiş, 3'ü erkek, 17'si kadın olmak üzere toplam 20 morbid obez hasta dahil edilmiştir. Çalışmaya alınan hastaların preop ve postop 3. ve 6. aydaki antropometrik ölçümleri, biyokimyasal bulguları, komorbiditeleri, komplikasyonları ve beslenme alışkanlıkları değerlendirilmiştir. İstatistiksel anlamlılık için, Friedman Test ve Cochran's Q test kullanılmış olup, p<0,05 anlamlı kabul edilmiştir. Bulgularımızda, hastaların preop dönemde ağırlığı 126,01±23,52 kg iken, postop 6. ayda ortalama 34 kg kaybettikleri saptanmış olup, ilk 6 ayda fazla kilo kaybı oranın %60,2 olduğu tespit edilmiştir. Hastaların yağ kütlelerinin preop dönemde 56,91±7,64 kg'dan, postop 6. ayda 40,21±8,40 kg'a düştüğü bulunmuştur (<0,0001). Kalsiyum, Açlık kan şekeri, HbA1C, LDL, ALT, AST, total kolesterol, trigliserit, folik asit ve ürik asit değerlerindeki farklılığın istatistiksel olarak anlamlı olduğu ve yüksek olan değerlerin normal sınırlara düştüğü kaydedilmiştir. Hastalarda görülen mevcut 21 komorbiditeden 15'inin, postop 6. ayda iyileştiği belirtilmiştir. Çalışmamızdaki hastaların sleeve gastrektomi sonrası, bariyatrik cerrahiye uygun olarak beslenmeleri sonucunda, kilo kaybettikleri, biyokimyasal bulguların düzeldiği ve komorbiditelerin iyileştiği saptanmıştır.Obesity has become an increasing health problem across the globe. Bariatric Surgery, not only ensures to maintain the chronic weight loss, but also substantially prevents the progression of comorbids linked to obesity. Sleeve Gastrectomy, one of the methods of bariatric surgery, has become a method that grew in reliability and practiced frequently in recent years. Therefore, in our study, postoperative 6 months, the interperiod effect of the nutritional program that is practiced on patients according to bariatric surgery rules is researched. The study is conducted in Bursa Uludağ University Faculty of Medicine Hospital, General Surgery Clinic, between the dates of September 2016 and June 2017. 20 Morbid Obes patients who had SG operation, between the ages of 18-65, 3 males and 17 females are included in our study. Anthropometric measurements, biochemical findings, comorbids, complications and dietary habits of the preoperative and postoperative patients during the 3rd and 6th months are evaluated. For the sake of statistical significance, Friedman Test and Cochran's Q Test are used, and p<0,05 is accepted. In our findings, while the weights of the patients during the preoperative period was 126,01±23,52 kgs, during the postoperative 6th month, it was determined that, the patients had lost 34 kgs, and it was confirmed that the EWL was %60,2 during the first six months. It is found that the fat mass of the patients decreased during the preoperative period from 56,91±7,64 to 40,21±8,40 in postoperative 6th month (<0,0001). Differecences of the levels of calcium, preprandial blood glucose, HbA1C, LDL, ALT, AST, total cholesterol, triglyceride, folic acid and uric acid are statistically accepted and it is recorded that high levels have decreased to normal. It is pointed out that the 15 of 21comorbids present in patients have recovered in the postoperative 6th month. It is determined that after Sleeve Gastrectomy, as a result of a proper diet in accordance with bariatric surgery, patients have lost weight, biochemical findings have improved and comorbids have recovered.
