Unilateral transtibial amputelerin protez kullanım deneyimi ve memnuniyeti

Abstract

Bu çalışma, alt ekstremite unilateral amputelerin protez kullanım deneyimlerini ve memnuniyetlerini araştırmak; katılımcıların demografik bilgileri, amputasyon ve proteze ait özellikleri ile karşılaştırmak amacıyla planlandı. Çalışmaya unilateral transtibial ampute, 18-75 yaş arası, en az 1 yıldır dizaltı protezi kullanan ve gönüllü olarak katılım sağlayan 31 birey dahil edildi. Çalışmaya katılan bireylerin genel bilgileri, hasta bilgi ve değerlendirme formu ile kaydedildi. Bireylerin protez memnuniyetini ölçmek için Protez Memnuniyet Anketi (PMA), ile protez deneyimini ölçmek için Trinity Amputasyon ve Protez Deneyim Ölçeği (TAPES) uygulanmıştır. TAPES ile bireylerin psikososyal, aktivite kısıtlama ve protez memnuniyetleri ve alt ölçekleri değerlendirilmiştir. PMA ölçek skorları genel bilgilerle karşılaştırıldığında herhangi anlamlı bir veriye rastlanmadı. Protez memnuniyet ve deneyimi TAPES'e göre incelendiğinde, katılımcıların yaş, boy, kilo değerleri ile protez genel uyumu arasında negatif yönde orta şiddette bir anlamlı korelasyon saptandı. Kadınlarda aktivite kısıtlama ölçek puanlarının, erkeklere oranla düşük skorda olduğu belirlendi (p<0.05). Amputayon sebepleri karşılaştırıldığında katılımcıların aktivite kısıtlama, atletik kısıtlama, sosyal kısıtlama alt ölçek ortalamaları; vasküler nedenli amputelerde, travmatik ve diğer nedenli amputelere göre daha yüksek çıktı (p<0.05). Ayrıca protez ağırlık memnuniyeti vasküler amputelerde travmatik ve diğer amputelere göre daha düşük olduğu görüldü (p<0.05). Kısıtlamaya uyumun skoru; vasküler ampute grupta, travmatik ve diğer nedenli gruplara göre daha yüksek bulundu (p<0.05). Katılımcıların eğitim seviyesi arttıkça, genel uyumlarının arttığı, aktivite kısıtlamalarının ise düştüğü bulundu (p<0.05). Amputelerin kullandığı farklı protez çeşitlerine göre TAPES ve PMA skorlarında fark bulunmadı. Sonuç olarak; katılımcıların protez memnuniyeti ve deneyiminde; yaş, kilo, cinsiyet, amputasyon sebebi ve eğitim seviyesinin etkili olduğu görüldü.This study was aimed to investigate prosthesis use experiences and satisfaction of lower extremity unilateral amputees and it was planned to compare the demographic information of the participants with the property of their amputation and prosthesis. 31 individuals included to the study who had unilateral transtibial amputation, aged 18-75years, using above knee prosthesis for at least 1 year and participating voluntarily. The characteristics of the individuals who are participating in the study were recorded with the patient information and evaluation form. 15 questions asked to admit Prosthesis satisfaction questionnaire (PMA) scores was used to measure prosthesis satisfaction of individuals, and Trinity amputation and prosthesis inspection scale (TAPES) was applied online to measure prosthesis user's experience and satisfaction. Psychosocial, activity restriction and prosthetic satisfaction and subscales of individuals were evaluated with TAPES. No significant data were found in the PMA scale scoring. When prosthesis satisfaction and experience were analyzed according to TAPES, a moderately significant negative correlation was found between age, height, weight values of the participants and the general harmony of the prosthesis. It was determined that the activity restriction scale scores in women were lower than men (p<0.05). When the participants compered by causes of amputation; activity restriction, athletic restriction and social restriction subscale averages was higher in vascular amputees than traumatic and other caused amputees (p<0.05). In addition, prosthetic weight satisfaction score was found lower in vascular amputees than in traumatic and other amputees (p<0.05). Score of adjustment to the limitation; It was found to be higher in the vascular amputee group than in the traumatic and other cause groups (p<0.05). It was observed that higher leve educated participant's general compliance of the prosthesis is better compared to other (p<0.05). There was no difference in TAPES and PMA scores according to different prosthesis types used by amputees. As a result; age, weight, gender, cause of amputation and education level were found to be effective to prosthesis satisfaction and experience of the participants.
