İnmeli hastalarda mekanik hippoterapi cihazı ile yapılan egzersizin postural kontrol ve denge üzerine etkisi

Abstract

İnme sonrası meydana gelen denge ve gövde kontrol bozuklukları asimetrik yürüme paternine, düşme riskinin artmasına ve yaşam kalitesinin bozulmasına neden olur. Hippoterapide atın yürümesiyle ortaya çıkan ritmik ve tekrarlayıcı hareketler insanın yürümesine benzer özellikler göstererek postürün, dengenin ve kas kuvvetinin gelişmesine yardımcı olur. Hippoterapi merkezlerinin az olması, maliyet, erişim, iklim ve çevresel koşullar ile hastaların atlardan korkması gibi zorluklar nedeniyle alternatif olarak mekanik hippoterapi cihazları geliştirilmiştir. Araştırmamız mekanik hippoterapinin inmeli hastalarda postural kontrol ve denge üzerine etkisini incelemek ve konvansiyonel egzersizlerle kıyaslamak amacıyla planlandı. Çalışmaya dahil edilen 30 hasta randomize olarak iki gruba ayrıldı: her iki gruba uygulanan 45 dk'lık geleneksel tedaviye ek olarak Hippoterapi Grubu (HG) 15 dk mekanik hippoterapi cihazı ile egzersiz, Kontrol Grubuna (KG) ise 15 dk oturma pozisyonunda denge ve postural kontrol egzersizleri verildi, haftada 5 kez, 4 hafta boyunca uygulandı. Katılımcılar Berg denge ölçeği (BDÖ), BIODEX cihazı ölçümleri, Gövde Bozukluk Ölçeği (GBÖ), Fonksiyonel Bağımsızlık Ölçeği (FBÖ), Zamanlı Kalk Yürü testi (ZKYT), Fugl-meyer ölçeği (FM), Fonksiyonel uzanma testi (FUT) ve SF-36-Kısa form değerlendirmeleri yapıldı. Çalışma sonucunda elde ettiğimiz verilere göre her iki grupta da uygulanan tedavilerden önemli ölçüde fayda sağlandı. Mekanik hippoterapi grubunda FM, GBÖ, FUT, ZKYT, 5OKT, SF-36 skorlarında kontrol grubuyla karşılaştırıldığında istatistiksel olarak daha fazla iyileşme sağladığı görüldü (p<0,05). Sonuç olarak, her iki grupta da uygulanan tedaviler etkili olmuş ancak mekanik hippoterapi cihazıyla yapılan egzersizlerin denge ve postural kontrolün geliştirilmesinde konvansiyonel egzersizlerden daha etkili olduğu gösterilmiştir.Reduced postural control and balance stability after stroke results gait disturbances such as asymetric weight bearing, risk of falling and imbalances. Hippotherapy uses equine movements. The horse's gait provides rhythmic and repetitive movements similar to human walking, thereby giving them training opportunities to improve posture, balance, and strength. So that we investigated the effects of exercises by mechanical hippotherapy device on postural control and balance in patients with stroke. We randomly allocated 30 patients with stroke in two groups. The hippotherapy group (HG) (n=15) received conventional rehabilitation for 45 min/day following by use of a hippotherapy device for 15 min/day, 5 times/week for 4 weeks. The control group (CG) (n=15) received conventional rehabilitation for 45 min/day, following by postural balance exercises 15 min/day 5 times/week for 4 weeks. All subjects was assessed by Berg balance scale (BBC) and BIODEX, Timed up and go test (TUG), SF-36 Short form, Functional reach test (FRT), Functional independence measurement (FIM), Fugl-meyer, Trunk control test (TCT), Trunk impairment Scale (TIS). At the end of study FM, TIS, FRT, TUG, 5TSST and SF-36 scores improved statistically significant in mechanical hippotherapy group (HG) compared to control group (CG) (p<0,05). As a result of study we found that exercises by mechanical hippotherapy device is beneficial on postural control and balance parameters in patients with stroke.
