Bidireksiyonel kavapulmomer anastomoz uygulanan tek ventrikül fizyolojili hastaların preoperatif nötrofil ve lenfosit dağılımının postoperatif komplikasyonlar üzerine etkisinin değerlendirilmesi

Abstract

Bidireksiyonel kavopulmoner anastomoz ameliyatları fonksiyonel tek ventriküllü konjenital kalp hastalarında uygulanan cerrahi bir yöntemdir. Hastalarda çoğunlukla konjestif kalp yetmezliği ve hipoksemi görülmektedir. Hastaların postoperatif takibinde bazı komplikasyonlar görülebilmektedir. Bu çalışmada amacımız, bidireksiyonel kavopulmoner anastomoz uygulanan tek ventrikül fizyolojisine sahip hastalarda ameliyat öncesi ölçülen lenfosit ve nötrofil değerlerinin postoperatif gelişen komplikasyonlar üzerine etkisinin olup olmadığını araştırmaktır. Çalışmaya 2013-2016 tarihlerinde Bidireksiyonel Kavopulmoner Anostomoz uygulanan 31 hasta dahil edildi. Çalışma retrospektif olarak yapılmıştır. Hastaların operasyon öncesi, birinci, ikinci ve üçüncü gün nötrofil ve lenfosit parametrelerine bakıldı. Hastaların postoperatif yoğun bakım ünitesinde (YBÜ) solunum düzeyi, vücüt ısısı takibi, drenaj miktarı ekstübasyon zamanı, YBÜ kalış zamanı, hastane taburculuk zamanı ve periton diyaliz uygulanma durumu bulguları takip edildi. Bu bulgulara dayanarak, preoperatif nötrofil değerlerinin postoperatif birinci günden itibaren anlamlı olarak yükseldiği (p<0.05), preoperatif lenfosit değerlerinin postoperatif birinci günden itibaren anlamlı olarak düştüğü (p<0.05) tespit edilmiştir. Ancak postoperatif takip edilen hastaların vücut ısısı, drenaj miktarı, solunum düzeyi ve yoğun bakımda kalış süresi ile farklı zamanlarda ölçülen lenfosit ve nötrofil değerlerinin ilişkisi anlamsız (p>0.05) bulunmuştur.Bidirectional cavopulmonary anastomosis operations are a surgical procedure performed in functional single ventricular congenital heart disease. Patients often have congestive heart failure and hypoxemia. Some complications can be seen in the postoperative follow-up of the patients. In this study, we aimed to investigate the effect of pretreatment lymphocytes and neutrophils on the postoperative complications in patients with single ventricular physiology with bidirectional cavopulmonary anastomosis. Thirty-one patients who underwent Bidirectional Cavo-pulmonary Anastomosis between 2013 and 2016 were included in the study. The study was performed retrospectively. Neutrophil and lymphocyte parameters were evaluated preoperatively, on the first, second and third day of the patients. Respiratory level, body temperature, extubation time, drain time, hospital discharge time and peritoneal dialysis status were followed in postoperative intensive care unit (ICU) of the patients. Based on these findings, it was determined that preoperative neurotrophic values were significantly elevated from the first postoperative day (p <0.05) and preoperative lymphocyte levels decreased significantly from postoperative first day (p <0.05). However, it has been found that it is meaningless to measure the body temperature, drainage amount, respiration level and duration of respiration and neutrophil values between the lymphocytes and the intensive care unit at different times (p> 0.05).
