Epitelyal over kanserli hastalarda sistemik inflamatuar indeks parametrelerinin platin direnci üzerine, nüks özelliklerine ve sağkalımlara etkisi

Abstract

Over kanseri gelişmiş ülkelerde en sık görülen ikinci, gelişmekte olan ülkelerde en sık üçüncü sırada görülen jinekolojik kanserdir. Diğer yandan, jinekolojik kanserler içinde en ölümcül olanıdır. Over kanserinin sağkalımlar üzerine etkisi olan majör prognostik faktörleri yaş, rezidual doku volümü, performans skoru ve histolojidir. Çalışmamızda, 154 EOK hastası retrospektif olarak analiz edildi. Hastaların FIGO evreleri, ECOG skorları, cerrahi tipi, histoloji, lokalizasyon, grade, aldıkları tedaviler, CA125 değerleri, tedavi öncesi hemogram parametreleri kaydedildi. Daha sonra, hastalarda klinikopatolojik faktörler ve tedavi öncesi sistemik enflamatuar markırların PFS ve OS üzerine prognostik faktör olarak etkileri olup olmadığının ortaya konulması amaçlandı. Ayrıca, platin direnci üzerine etkili olabilecek prediktif faktörler de değerlendirildi. Tedavi öncesi tam kan sayımındaki nötrofil, lenfosit, trombosit değerlerine göre NLR, PLR ve SII değerleri hesaplandı, bu değerlere göre de hastalar sırasıyla düşük ve yüksek NLR (?3 vs. >3), PLR (?194 vs >194) ve SII (?973.1 vs. >973.1) olarak sınıflandırıldılar. Tüm istatistiksel analizler SPSS 24.0 (SPSS Inc., Chicago,IL, USA) sürümü kullanılarak yapıldı. Klinikopatolojik özellikler, NLR, PLR ve SII ile sağkalımlar arasındaki ilişki tek değişkenli ve çok değişkenli analizler ile cox orantısal hazard model kullanılarak değerlendirildi. Platin direnci varlığı üzerine etkili faktörlerin belirlenmesi için binary logistik regresyon analizi uygulandı. 95% güven aralığı (CI) sağkalım zamanı ve her bir bağımsız faktör arasındaki ilişkiyi belirtmek için kullanıldı. Tüm p değerleri 2 yanlı olup, 0.05'e eşit ya da altındaki değerler istatistiksel olarak anlamlı kabul edildi. PFS için yapılan tek değişkenli analizde, FIGO evresi, ECOG PS durumu, primer tümör yerleşimi, yapılan cerrahi yöntemi, platin duyarlılığının olup olmaması, NLR, PLR ve SII prognostik faktörler olarak bulundu. Hastalarda yaş, evre, ECOG PS, cerrahi tipi ve platin duyarlı olup olmaması, NLR, PLR ile OS arasında anlamlı ilişki vardı. PFS için çok değişkenli analizde ise, FIGO evresi, histopatoloji, cerrahi tipi ve platin direnci varlığı bağımsız prognostik göstergelerdi. OS için aynı analiz yapıldığında ise, ECOG PS, cerrahi tipi ve platin direnci varlığı bağımsız prognostik faktörlerdi. NLR, PLR ve SII'lerin sağkalımlar üzerine prognostik etkileri tek değişkenli analizde gösterilse de çok değişkenli analiz ile bağımsız etkileri gösterilemedi. Platin direncini öngörmede histopatolojik tip, PLR ve cerrahi tip bağımsız prediktif faktörlerdi. Sonuç olarak, çalışmamızda NLR, PLR ve SII'nın EOK'li hastlarda prognostik faktör oldukları gösterilmiştir. Ayrıca, EOK'li hastalarda takipte en önemli prognostik faktörlerden biri olan platin direncini öngördürmede, yüksek PLR, berrak hücreli histoloji varlığı ve suboptimal cerrahi ya da inoperabıl olmak bağımsız prediktif faktörler olarak bulunmuştur.Ovarian cancer is the second most frequent gynecologic cancer in developed countries and the third most frequent gynecologic cancer in developing countries. On the other hand, it is the most mortal cancer of gynecologic cancers. Major prognostic factors of ovarian cancer which affect survivals are age, residual tissue volume, performance score and histological type. In our study, a total of 154 EOC patients were retrospectively analyzed. Patients' information about FIGO stages, ECOG scores, surgery types, histological types, tumors' localization, histological grades, treatments they had, CA 125 values, hemogram parameters before treatment were recorded. And then it is aimed to show whether there were effects of clinicopathological factors and before treatment systemic inflamatory markers on PFS and OS as prognostic factors. Predictive factors were also evaluated which could affect platinum resistance. NLR, PLR and SII values were calculated by using before treatment counts of neutrophiles, lymphocytes and trombocytes. According to these values these were classified as low and high NLR (?3 vs. >3), PLR (?194 vs. >194) and SII (?973.1 vs. >973.1) respectively. All statistical analyses were made by using SPSS version 24.0 (SPSS Inc., Chicago, IL, USA). Relation between clinicopathological characteristics, NLR, PLR and SII with survivals were evaluated by using univariate analysis, multivariate analysis and cox proportional hazards model. Binary logistic regression analysis is used for detection of factors which could affect platinum resistance. 95% CI is used to indicate relation between survival time and each independent factors. All p values were two sided and it was accepted statistically significant if p values were equal or less-than 0.05. FIGO stage, ECOG PS, primary tumor localisation, surgery type, the existence of platinum resistance, NLR, PLR and SII were found as prognostic factors in univariate analysis for PFS. Significant relations between age, stage, ECOG PS, surgery type, the existence of platinum resistance, NLR and PLR with OS were detected. FIGO stage, histopathological type, surgery type and the existence of platinum resistance were independent prognostic factors in multivariate analysis for PFS. On the other hand it is detected that ECOG PS, surgery type and the existence of platinum resistance were independent risk factors in similar analysis for OS. Prognostic effects of NLR, PLR and SII for survival were detected in univariate analysis whereas they were not detected in multivariate analysis. Hystopathological type, PLR and surgery type were independent predictive factors. In conclusion it is proved that NLR, PLR and SII are prognostic factors in EOC patients in our study. It is also proved that for projection of platinum resistance which is one of the most important prognostic factors in clinical follow-up, high PLR, clear cell histology and suboptimal surgery/inoperable status are independent risk factors in EOC patients.
