Cerrahi ve transkateter atriyal septal defekt kapama operasyon öncesi ve sonrası kardiyak bulguların renkli doppler transtorasikekokardiyografi ile değerlendirilmesi

Abstract

Atrial septal defektler en sık karşılaşılan konjenital kalp hastalıklarından biridir. Günümüzde hastaların tanısı ve takibinde ekokardiyografi kullanılmaktadır. Endikasyonu olan hastalarda tedavi cerrahi ya da transkateter yöntemler ile yapılabilir. Çalışmamızın amacı, cerrahi ya da transkateter atrial septal defekt kapama yöntemlerinin kardiyak yapı ve fonksiyonları üzerindeki etkisini, defektin çapı ile ilişkilendirerek operasyon öncesi, operasyon sonrası 3. ay ve operasyon sonrası 12. aydaki bulguları renkli Doppler transtorasik ekokardiyografi görüntüleri aracılığıyla karşılaştırarak ortaya koymaktır. Çalışmamıza Ocak 2013-Aralık 2019 tarihleri arasında hastanemiz kalp ve damar cerrahisi ve pediyatrik kardiyoloji kliniklerinde tedavileri gerçekleştirilen 70 hasta dahil edilmiştir. Cerrahi onarım 40 (Grup 1), transkateter kapama yapılan 30 hasta (Grup 2) retrospektif olarak incelenmiştir. Hastaların atrial ve ventriküler morfolojik değerlendirmeleri transtorasik ekokardiyografi ile yapılmıştır. Gruplar arasındaki ekokardiyografik M-mod bulguları irdelenmiştir. Operasyon sonrası 3. aydaki kardiyak anotomi, geometri ve fonksiyonlardaki düzelme göz önüne alındığında; RA major aks (Grup 1: -7,17±5,44; Grup 2: -4,13±3,79; p=0,008), RA volüm (Grup 1: -4,02±2,70; Grup 2: -2,03±2,17; p=0,001), IVSd (Grup 1: -1,00 (-1,00-0,75); Grup 2: 0,00 (-1,00-1,43); p=0,049), IVSs (Grup 1: -1,00 (-2,00-0,00); Grup 2: 0,00 (-1,00-1,00); p=0,006), RVEDd (Grup 1: -8,17±4,31; Grup 2: -4,61±3,67; p=0,001) parametrelerinde cerrahi grupta daha hızlı düzelme olmuştur. Operasyon sonrası 12. aydaki kardiyak anotomi, geometri ve fonksiyonlardaki düzelme göz önüne alındığında RA major aks (Grup 1: - 8,00±5,39 Grup 2: -4,00±5,09; p=0,002), RA volüm (Grup 1: -3,28±2,50; Grup 2: - 1,41±3,48; p=0,011) ve RVEDd (Grup 1: -9,15±5,31; Grup 2: -5,82±4,48; p=0,007) için cerrahi grupta daha hızlı düzelme olmuştur. Atrial septal defektlerin tedavisi cerrahi ya da transkateter yolla başarılı bir şekilde yapılabilmektedir. Çalışmamız verilerine göre cerrahi kapama yönteminin sağ ventrikül diastol sonu çaplarını (RVEDd) transkateter yönteme göre daha başarılı bir şekilde erken dönemde düzelttiği görülmüştür.Atrial septal defects are among one of the most common congenital heart diseases. Currently, echocardiography is used for the diagnosis and follow-up of patients. Surgical or transcatheter methods may be used for the treatment in patients with indications. The aim of our study is to investigate the effects of surgical and transcatheter closure methods on cardiac structure and myocardial functions with respect to the diameter of the defect and transthoracic color Doppler echocardiographic findings at the preoperative, postoperative 3rd and 12nd months. Seventy patients who were treated in cardiovascular surgery and pediatric cardiology clinics of our hospital between January 2013-December 2019 were included in our study. Fourty patients (Group 1) who underwent surgical repair and 30 patients (Group 2) who underwent transcatheter closure were retrospectively examined. Atrial and ventricular morphological evaluations were performed with transthoracic echocardiography. Echocardiographic M-mode findings were examined and compared. Considering the improvement in cardiac anatomy, geometry and functions in the 3rd month after the operation, the parameters that showed faster recovery in the surgical repair group were RA major axis (Group 1: -7,17±5,44; Group 2: -4,13±3,79; p=0,008), RA volume (Group 1: - 4,02±2,70; Group 2: -2,03±2,17; p=0,001) IVSd (Group 1: -1,00 (-1,00-0,75); Group 2: 0,00 (-1,00-1,43); p=0,049), IVSs (Group 1: -1,00 (-2,00-0,00); Group 2: 0,00 (-1,00-1,00); p=0,006), RVEDd (Group 1: -9,15±5,31; Group 2: -5,82±4,48; p=0,007). Corresponding values in the 12th month after the operation, in the surgical repair group were RA major axis (Group 1: -8,00±5,39 Group 2: -4,00±5,09; p=0,002), RA volume (Group 1: -3,28±2,50; Group 2: -1,41±3,48; p=0,011) and RVEDd (Group 1: -9,15±5,31; Group 2: -5,82±4,48; p=0,007). Treatment of atrial septal defects can be successfully performed surgically or percutaneously. According to the findings of our study, it has been observed that the surgical closure method enhanced the recovery of the right ventricular end-diastolic diameters (RVEDd) more successfully at an early stage than the transcatheter method.
