Ratlarda hipoglosso-fasiyal sinir anastomozunda trombositten zengin fibrin (TZF) membranın fonksiyonel ve histopatolojik iyileşmeye etkisi

Abstract

Hipoglossal-fasiyal sinir anastomozu, fasiyal fonksiyonlarda fonksiyonel bir iyileşme elde etmek için hipoglossal ve fasiyal sinir güdüklerini yaklaştırmayı amaçlar. Hipoglosso-fasiyal anastomozda lokal trombosit zengin fibrin (TZF) uygulamasının fonksiyonel ve histopatolojik sonuçlarını araştırmayı amaçladık. Toplam, kırk adet dişi Sprague Dawley sıçan kullanıldı. TZF elde etmek için sekiz sıçan kullanıldı. Kalan 32 sıçan eşit olarak 4 gruba ayrıldı; i-sham grubunda diseksiyonla hipoglossal ve fasiyal sinirler ortaya çıkarıldı; ii- kontrol grubunda hipoglossal sinirin distal ve fasiyal sinirin proksimal kesik uçları başka herhangi bir müdahale olmadan yaklaştırıldı; iii- PS grubunda hipoglossal sinirin distal ve fasiyal sinirin proksimal kesik uçları sütüre edildi; ve iv- PS + TZF grubunda hipoglossal sinirin distal ve fasial sinirin proksimal kesik uçları dikişlerle yaklaştırılıp ve TZF membran ile çevrelenmiştir. Fonksiyonel ve histopatolojik sonuçlar üç ay sonra değerlendirildi. Yüz simetrisi nazal alfa açısı ile değerlendirildi. PS+ TZF grubunda ilk iki ay fasiyal simetri düzelmesi ile gösterilen fonksiyonel iyileşme daha hızlı gerçekleşti (kontrol p=0.006; PS p=0.002). TZF membran lokal uygulamasının anlamlı ölçüde iyileşme sürecini hızlandırdığını ortaya koymaktadır. TZF membran uygulamasının iyileşmeyi artırmasına rağmen 3. ayın sonunda fasiyal simetri düzeyinde iyileşmede anlamlı farklılık yaratmadığı görüldü (p=0.607;p>0,05). Elektrofizyolojide, 0.4mA uyarı ile amplitüdü PS + TZF grubu lehine anlamlı olarak farklıydı (p = 0.024). Akson sayısı, yoğunluğu, akson ve miyelin kalınlığı açısından gruplar arasında anlamlı fark yoktu (p>0.05). Sonuç olarak, TZF uygulaması elektrofizyolojik ölçümlerle tespit edildiği gibi hipoglossofasiyal sinir anastomozundan sonra yüz fonksiyonlarının düzelmesinde artışa yol açmaktadır.Hypoglossofacial nerve anastomosis aims to approximate hypoglossal and facial nerve stumps to obtain a functional recovery in the facial functions. We aimed to investigate the functional and histopathologial outcomes of local application of platelet rich fibrin (PRF) on the hypoglosso-facial anastomosis. Eight rats were used to obtain PRF. The remaining 32 rats were equally divided into 4 groups as follows; i-the hypoglossal and facial nerves were exposed by dissection in the sham group; ii-the cut ends of distal hypoglossal nerve and proximal facial nerve were approximated without any other intervention in the control group; iii-the cut ends of distal hypoglossal nerve and proximal facial nerve were approximated with sütüres in the primer suturation(PS) group; and iv-the cut ends of distal hypoglossal nerve and proximal facial nerve were approximated with sütüres and encircled with PRF in the PS+PRF group. The functional and histopathological outcomes were assessed after three months. Facial symmetry was evaluated in the nasal alpha direction. In the PS+PRF group, functional improvement with facial symmetry improvement was faster in the first two months (control p=0.006;PS p=0.002) with no significant difference at the end of 3 months ( p=0.607;p>0.05). On electrophysiology, an amplitude of 0.4mA stimulus was significantly different in favor of PS+PRF group (p=0.024). There was no significant difference between the groups regarding the number, density and settings of axons, and myelin thickness (p>0.05). In conclusion, PRF application leads to an improvement of recovery of the facial functions after hypoglossofacial nerve anastomosis.
