Çocuk allerjik rinit olgularında adenotonsiller hipertrofi ile seröz otit sıklığı ve bunların ameliyat prognozunun belirlenmesi

Abstract

Alerjik rinit (AR), hapşırma, burun tıkanıklığı, berrak rinore ve burun kaşıntısı semptomlarıyla ortaya çıkan atopik bir hastalıktır. Tüm dünyada ciddi bir sağlık problemi olan alerjik rinit, çocukluk çağının kronik hastalıklarının en sık görülenlerinden biridir. Özellikle sanayileşmiş ülkelerde daha sık görülmekle birlikte bu ülke yetişkinlerinin %10-30'u, çocukların ise %40'ının AR'den etkilendiği saptanmıştır. 98 ülkede 1,2 milyon çocukla yapılan Uluslararası Çocukluk Çağı Astım ve Alerji Çalışmasının (ISAAC) sonuçlarına göre alerjik rino-konjunktivit sıklığı 6-7 yaş grubunda %0,8-14,9, 13-14 yaş grubunda %1,4-39,7 saptanmıştır. Yine ISAAC çalışmasına göre gelişmekte olan ve az gelişmiş ülkelerde daha fazla olmak üzere AR sıklığının artmakta olduğu görülmüştür. AR'nin genetik ve çevresel faktörlerin etkileşimiyle ortaya çıkan multifaktöryel bir hastalık olduğu kabul edilmektedir. Yalnızca hastalığın oluşumu değil, hastalığın şiddeti de bu faktörler ile belirlenir. AR'nin 30'dan fazla gen ile ilgisi olduğu bilinmektedir. Son yıllarda alerjik hastalıkların sıklığındaki artışa çevresel faktörlerdeki değişikliklerin neden olabileceği düşünülmektedir. AR sıklığı ile birlikte adenoid hipertrofisi ve effüzyonlu otitis medianın da arttığı çalışmalarla ispatlanmış, medikal ve cerrahi yöntemlerin başarısı ve birbirine üstünlüğü olup olmadığı ile ilgili birçok veri ortaya konmaya çalışılmıştır. Çalışmamız dünyada sıklığı giderek artan, kısa ve uzun vadede ciddi sorunlara yol açan, yaşam kalitesini önemli ölçüde etkileyen çocuk alerjik rinit olgularında adeno-tonsiller hipertrofi ile seröz otit sıklığının saptanması ve prognostik faktörlerin belirlenmesi amacıyla yapıldı. Çalışmanın evrenini Covid-19 pandemi öncesindeki son 3 yılda genel pediatri polikliniğimize başvuran alerjik riniti olan 1668 çocuk oluşturdu. Çalışmamızda AR görülme sıklığı erkek çocuklarda kız çocuklara göre daha yüksek bulundu. Hastalarda görülen semptomlar en çoktan en aza sırasıyla burun tıkanıklığı (%99,9), geniz akıntısı (%99,5), hapşırık (%95), öksürük (%84) ve gözlerde kızarıklık (%13,4) olarak saptandı. Serum eozinofilisi ile adenotonsiller hipertrofisi olan çocukların ameliyat olma durumları arasındaki ilişki incelendiğinde adenoidektomi/tonsillektomi olan çocuklarda serum eozinofilisi, olmayanlara göre istatistiksel olarak anlamlı yüksek bulundu (p=0,036). Adenotonsiller hipertrofi olup ameliyat olan çocukların serum D vitamini düzeyleri arasında istatistiksel olarak anlamlı bir sonuç bulunamadı (p=0.629). Olguların izlem süresi ortalaması 11,79 ay (en fazla 71 ay) olarak saptandı. Ameliyat olan çocukların (n=282, %16,9) ise izlem süresi ortalaması 9 ay, ameliyat olmayanların (n=1386, %83,1) ise takipte kalma süresi ortalaması 4 ay saptandı. Adenotonsiller hipertrofisi olan çocukların izlem sürelerinin daha uzun olduğu görüldü; ancak izlem süresi ile adenoidektomi/tonsillektomi arasında istatistiksel anlamlı bir ilişki saptanmadı. İzlemleri boyunca şiddetli adenotonsiller hipertrofisi dışında takip edilen hastalarda medikal tedavi denendi ve medikal tedavinin cevabı gözlendi. Buna rağmen adenotonsiller hipertrofisi hala ciddi boyutlarda olan çocuklar ameliyat edildi. Seröz otit saptanan 636 (%38,1) çocuktan 142 (%22,3)'sinde adenoidektomi/tonsillektomi saptandı. Antihistaminik, nazal steroid ve montelukastı içeren medikal tedavilerden hiçbirinin adenoidektomi/tonsillektomi için önleyici olmadığı saptandı. Antihistaminik kullanan 1587 (%95,1) çocuğun 269(%17)'u ameliyat olmuş kullanmayan çocukların ise %16'sı ameliyat olmuş olarak saptandı. Antihistaminik alerjik rinitte en sık kullanılan medikal tedavi olmakla birlikte ameliyat olmayı engellemediği saptandı. Nazal steroid kullanan 1491 (%89,4) çocuğun 273 (%18,3)'ü ameliyat olduğu saptandı. Nazal steroid kullanmayanlarda ameliyat oranı %5,1, nazal steroid kullananlarda ameliyat oranı %18,3 olmakla birlikte istatistiksel olarak önemli derecede farklı bulundu (p<0.05). Nazal steroidin ameliyat olmayı engelleyen bir medikal tedavi olmadığı, ancak seröz otit ve adenoid/tonsiller hipertrofisi olanlarda daha sık kullanıldığı saptandı. Montelukast kullanan 832 (%49,9) çocuğun 197 (%23,7)'sinin ameliyat olduğu saptandı. Montelukast kullanmayanlarda ameliyat oranı %10,2, montelukast kullananlarda ameliyat oranı %23,7 olmakla birlikte istatistiksel olarak önemli derecede farklı bulundu (p<0.05). Montelukastın ameliyat olmayı engelleyen bir medikal tedavi olmadığı, ancak alerjik rinit, seröz otit ve adenoid/tonsiller hipertrofisi olanlarda medikal tedavi seçeneği olarak kullanıldığı saptandı. İlaçların tekli kullanımı ve kombinasyonları karşılaştırıldığında ise hiçbirinin adenoidektomi/tonsillektomi ve seröz otiti engellemekte birbirine üstünlüğü olmadığı saptandı.Allergic rhinitis (AR) is an atopic disease that presents with symptoms of sneezing, nasal congestion, serous rhinorrhea and nasal itching. Allergic rhinitis, a serious health problem all over the world, is one of the most common chronic diseases of childhood. Although it is more common especially in industrialized countries, it has been determined that 10-30% of adults and 40% of children in this country are affected by AR. According to the results of the International Study of Childhood Asthma and Allergy (ISAAC) conducted with 1.2 million children in 98 countries, the frequency of allergic rhino-conjunctivitis is 0.8-14.9% in the 6-7 age group, 1.4-39,7% was detected in the 13-14 age group. Again, according to the ISAAC study, it was observed that the frequency of AR was increasing, especially in developing and underdeveloped countries. It is accepted that AR is a multifactorial disease caused by the interaction of genetic and environmental factors. Not only the occurrence of the disease, but also the severity of the disease is determined by these factors. AR is known to be associated with more than 30 genes. It is thought that the increase in the frequency of allergic diseases in recent years may be caused by changes in environmental factors. It has been proven by studies that adenoid hypertrophy and otitis media with effusion increase with the frequency of AR, and many data have been tried to be revealed about the success of medical and surgical methods and whether they are superior to each other. Our study was carried out to determine the frequency of adeno-tonsillar hypertrophy and serous otitis, and to determine the prognostic factors in pediatric allergic rhinitis cases, the frequency of which is increasing in the world, causing serious problems in the short and long term, and significantly affecting the quality of life. The population of the study consisted of 1668 children with allergic rhinitis who applied to our general pediatric outpatient clinic in the last 3 years before the Covid-19 pandemic. In our study, the incidence of AR was found to be higher in boys than in girls. The symptoms seen in the patients were found to be nasal congestion (99.9%), nasal discharge (99.5%), sneezing (95%), cough (84%) and redness in the eyes (13.4%), from the most to the lowest, respectively. When the relationship between serum eosinophilia and surgery status of children with adenotonsillar hypertrophy was examined, serum eosinophilia in children with adenoidectomy/tonsillectomy was found to be statistically significantly higher than those without (p=0.036). There was no statistically significant result between serum vitamin D levels of children with adenotonsillar hypertrophy who underwent surgery (p=0.629). The mean follow-up period of the cases was 11.79 months (maximum 71 months). The mean follow-up period was 9 months for the children who had surgery (n=282, 16.9%), and 4 months for those who did not have surgery (n=1386, 83.1%). The follow-up period was longer in children with adenotonsillar hypertrophy; however, no statistically significant relationship was found between the follow-up period and adenoidectomy/tonsillectomy. During the follow-up, medical treatment was tried in the patients who were followed up except for severe adenotonsillar hypertrophy, and the response of the medical treatment was observed. Despite this, children with severe adenotonsillar hypertrophy were operated on. Adenoidectomy/tonsillectomy was found in 142 (22.3%) of 636 (38.1%) children with serous otitis. It was determined that none of the medical treatments including antihistamine, nasal steroid and montelukast were preventive for adenoidectomy/tonsillectomy. It was determined that 269 (17%) of 1587 (95.1%) children using antihistamine had undergone surgery and 16% of children who did not use antihistamines were found to have had surgery. Although antihistamine is the most commonly used medical treatment in allergic rhinitis, it was found that it did not prevent surgery. It was determined that 273 (18.3%) of 1491 (89.4%) children using nasal steroids had surgery. Although the operation rate was 5.1% in those who did not use nasal steroids, and 18.3% in those who used nasal steroids, it was found to be statistically significantly different (p<0.05). It was found that nasal steroid is not a medical treatment that prevents surgery, but it is used more frequently in patients with serous otitis and adenoid/tonsillar hypertrophy. It was determined that 197 (23.7%) of 832 (49.9%) children using Montelukast had surgery. Although the rate of surgery was 10.2% in those who did not use montelukast, and 23.7% in those who used montelukast, it was found to be statistically significantly different (p<0.05). It was determined that Montelukast is not a medical treatment that prevents surgery, but it is used as a medical treatment option in patients with allergic rhinitis, serous otitis and adenoid/tonsillar hypertrophy. When the single use of drugs and their combinations were compared, it was found that none of them was superior to the other in preventing adenoidectomy/tonsillectomy and serous otitis.
