Vitiligoda anti-oksidan tiyol/disülfit homeostazının rolü: Yeni bir inflamatuvar belirteç

Abstract

Amaç: Tiyol ve disülfit seviyeleri total oksidan durumun bir belirteci olarak inflamatuvar hastalıklarda arttığı gösterilmiştir. Bu çalışmada, vitiligolu hastalarda tiyol ve disülfit kan seviyelerinin ya da tiyol/disülfit oranlarının artışını araştırmak ve yeni prognostik inflamatuvar belirteç olarak kullanılabilirliğini belirlemeyi amaçladık. Gereç ve Yöntem: Klinik olarak tanısı doğrulanmış 30 vitiligo hastasındaki ve 30 sağlıklı kontrol grubunda natif tiyol, total tiyol ve disülfit kan seviyeleri araştırıldı. Natif/total tiyol, disülfit/natif tiyol ve disülfit/total tiyol oranları iki grup arasında karşılaştırıldı. Vitiligolu hastalarda bu ölçümler ve Vitiligo Alan ve Şiddet İndeks değerleri arasındaki korelasyon da incelendi.Bulgular: Vitiligolu hastalarda kontrol grubuna oranla total tiyol seviyesi istatistiksel olarak anlamlı olmasa da, çarpıcı oranda (p=0,085), disülfit seviyeleri ise anlamlı oranda daha yüksekti (p=0,047) ancak natif tiyol seviyeleri benzerdi. Ayrıca disülfit/natif tiyol (p=0,049; p<0,05) ve disülfit/total tiyol (p=0,049) oranları da vitiligolu hastalarda kontrol grubuna oranla daha yüksek olarak saptandı. Vitiligo Alan ve Şiddet İndeks değerleri ile total tiyol (p=0,017) ve disülfit (p=0,042) seviyeleri arasında anlamlı korelasyon saptandı.Sonuç: Oksidatif stres ve doku inflamasyonunun bir sonucu olarak vitiligoda tiyol/disülfit dengesizliğini gösteren kan disülfit seviyelerindeki ve disülfit/natif tiyol, disülfit/total tiyol oranlarındaki artış anlamlı idi. Ayrıca disülfit ve total tiyol seviyeleri vitiligo şiddeti ile kesin pozitif bir korelasyon göstermekteydi.Objective: As a marker of total oxidant status the levels of thiol and disulphide have been shown to be increased in inflammatory diseases. In this study, we aimed to investigate the increment of the blood levels of thiol and disulphide or the thiol/disulphide ratios and to define their applicability as a new prognostic inflammatory marker in vitiligo patients. Material and Methods: Blood levels of native thiol, total thiol and disulphide were investigated in 30 vitiligo patients with a clinically proved diagnosis and 30 healthy controls. The ratios of native/total thiol, disulphide/native thiol and disulphide/total thiol were compared between two groups. The correlation between these measurements and Vitiligo Area and Severity Index values was also examined in vitiligo patients. Results: In vitiligo patients though not statistically significantly, the total thiol level was remarkably (p=0.085) higher, while disulphide levels were significantly higher (p=0.047) compared to the control group, however, the native thiol levels were similar. Moreover, the ratios of disulphide/native thiol (p=0.049); and disulphide/total thiol (p=0.049) were found to be higher in vitiligo patients compared to the control group. A significant correlation was observed between Vitiligo Area and ve Severity Index values and total thiol (p=0.017) and disulphide levels (p=0.042).Conclusion: The increase in the blood levels of disulphide and disulphide/native thiol, disulphide/total thiol that shows the imbalance in thiol/disulphide homeostasis as a result of the oxidative stress and tissue inflammation was significant. Besides, the disulphide and total thiol levels demonstrated a definitively positive correlation with the severity of vitiligo.
