Oral kemoterapi alan hastalarda telefon ile izlemin semptom düzeyi,tedaviye uyum ve yaşam kalitesine etkisi

Abstract

Bu araştırma oral kemoterapi kullanan hastalara uygulanan telefonla izlemin semptom düzeyi, tedaviye uyumu ve yaşam kalitesine etkisini belirlemek amacıyla yapılmış randomize kontrollü müdahale çalışmasıdır.Çalışmanın evrenini, 15.02.2021-15.09.2021 tarihleri arasında İstanbul ilinde bulunan bir vakıf üniversitesi hastanesinin ayaktan kemoterapi ünitesine başvuran ve oral kemoterapi kullanan kanser hastaları, örneklem büyüklüğünü ise bu hastalardan araştırmaya alınma şartlarını karşılayan, çalışmaya katılmaya gönüllü olan 25 müdahale ve 25 kontrol grubunda olmak üzere 50 hasta oluşturdu. Araştırmada veri toplama aracı olarak Sosyodemografik ve Hastalığa İlişkin Veri Toplama Formu, Oral Kemoterapiye Uyum Ölçeği, Edmonton Semptom Tanılama Ölçeği ve EORTC QLQ-C30 Yaşam Kalitesi Ölçeği kullanıldı. Müdahale grubunda olan hastalara, klinikte yüz yüze araştırmacı tarafından veri toplama araçları uygulanarak ilk görüşmede ön testler gerçekleştirildi. Ardından beşer gün aralıklarla (5., 10., 15. ve 20. gün) dört defa olmak üzere hastaların telefon ile izlemleri sağlandı ve dördüncü görüşmede son testler uygulandı. Kontrol grubunda olan hastalara ise, ilk görüşmede veri toplama araçları ile ön testler uygulandı. Kliniğin rutin uygulamaları dışında herhangi bir müdahale yapılmayıp yirmi gün sonra hastalar telefon ile aranarak son test verileri elde edildi. Araştırma da elde edilen bulgular değerlendirilirken, istatistiksel analizler için R vers. 2.15.3 programı (R Core Team, 2013) kullanıldı. Müdahale grubunda telefon ile izlem sonucu, hastaların ilaç uyumu puanın ve yaşam kalitesinin bulantı ve kusma, ağrı, iştah kaybı alanlarına ait puanlarının istatiksel olarak arttığı ancak semptom düzeyine ait puanlarının istatistiksel olarak anlamlı düzeyde azalmadığı bulundu. Kontrol grubundaki hastaların ise yaşam kalitesi, ilaç uyumu ve semptom düzeyi puanlarında istatistiksel olarak anlamlı fark saptanmadı.This research is a randomized controlled intervention study conducted to determine the effect of telephone follow-up on patients using oral chemotherapy on symptom level, adherence to treatment and quality of life. The population of the study is the outpatient chemotherapy unit of a foundation university hospital in Istanbul between 15.02. Cancer patients using oral chemotherapy, and the sample size consisted of 50 patients, 25 in the intervention group and 25 in the control group, who met the conditions of inclusion in the study and volunteered to participate in the study. Sociodemographic and Disease Data Collection Form, Oral Chemotherapy Adherence Scale, Edmonton Symptom Diagnostic Scale and EORTC QLQ-C30 Quality of Life Scale were used as data collection tools in the study. The patients in the intervention group were administered face-to-face data collection tools by the researcher in the clinic, and pre-tests were performed at the first interview. Afterwards, the patients were followed up by telephone four times at five-day intervals (5th, 10th, 15th and 20th days), and post-tests were applied at the fourth interview. Pre-tests with data collection tools were applied to the patients in the control group at the first interview. No intervention was made other than the routine practices of the clinic, and after twenty days, the patients were called by phone and the final test data were obtained. While evaluating the findings obtained in the research, R vers. The 2.15.3 program (R Core Team, 2013) was used. As a result of telephone follow-up in the intervention group, it was found that the patients' drug compliance score and the scores of the quality of life of nausea and vomiting, pain and loss of appetite increased statistically, but the scores of the symptom level did not decrease statistically. There was no statistically significant difference in the quality of life, drug compliance and symptom level scores of the patients in the control group.
