Subakromiyal sıkışma sendromu olan bireylerde birinci kosta mobilizasyonun etkileri

Abstract

Çalışmamızda subakromiyal sıkışma sendromu olan hastalarda egzersiz programına birinci kosta mobilizasyonu ekleyerek ağrı, fonksiyon ve omuz hareket açıları üzerine etkilerini araştırmayı amaçladık. Çalışma subakromiyal sıkışma sendromu tanısı almış ve dahil edilme kriterlerine uygun 48 olgu ile tamamlandı. Hastalar kontrol grubu ve mobilizasyon grubu olmak üzere gruplarda 24'er kişi olacak şekilde randomize kontrollü olarak iki gruba ayrıldı. Konservatif tedavi grubu kontrol grubu olarak atandı. Kontrol grubuna egzersiz, mobilizasyon grubuna ise egzersiz uygulamasına ek olarak birinci kosta mobilizasyonu uygulandı. Her iki gruba dahil olan bireyler üç hafta boyunca, haftada beş gün tedaviye alındı. Hastalara tedavi öncesi ve sonrasında, Vizuel Analog Skala, Omuz Ağrı ve Disabilite İndeksi, Quick-DASH (Q-DASH), Eklem Hareket Açıklığı değerlendirmesi yapıldı. Çalışma sonrası yapılan istatiksel analizde gruplar arası demografik bilgilerin karşılaştırılmasında anlamlı bir fark gözlemlenmedi. Eklem hareket açıklığı, Vizuel Analog Skala Gece ve Aktivite skoru ilk ve son test karşılaştırıldığında tedavi sonrası gruplar arasında mobilizasyon grubu yönünde anlamlı bir fark görüldü. Vizuel Anolog Skala İstirahat skoru, Quick-DASH ve Omuz Ağrı ve Disabilite İndeksi tedavi öncesi ve sonrası karşılaştırıldığında gruplar kendi içinde anlamlı bir düşüş göstermişken gruplar arası anlamlı bir sonuç bulunmadı. Çalışmanın sonucunda Subakromiyal sıkışma sendromu olan bireylerde konservatif tedavi programına ek olarak uygulanan birinci mobilizasyonunun fleksiyon, abduksiyon ve eksternal rotasyon açısını arttırıp gece ve aktivite ağrısını azalttığı bulundu. Subakromiyal sıkışma sendromu tedavisinde altın standart olarak görülen egzersiz tedavisine eklenmiş manuel terapi uygulamaları programına eklem hareket açıklığını arttırmak ve gece ve aktivite ağrısını azaltmak için dahil edilmesini öneriyoruz.In our study, we aimed to investigate the effects on pain, function and shoulder range of motion by adding first rib mobilization to the exercise program in patients with subacromial impingement syndrome. The study was completed with 48 cases diagnosed with subacromial impingement syndrome and meeting the inclusion criteria. The patients were divided into two randomized controlled groups, with 24 people in each group, the control group and the mobilization group. The conservative treatment group was assigned as the control group. Exercise was applied to the control group, and first rib mobilization was applied to the mobilization group in addition to the exercise application. Individuals in both groups were treated five days a week for three weeks. Visual Analogue Scale, Shoulder Pain and Disability Index, Quick-DASH (Q-DASH), Joint Range of Motion were evaluated before and after the treatment. In the statistical analysis performed after the study, no significant difference was observed in the comparison of demographic information between the groups. When the joint range of motion, Visual Analogue Scale Night and Activity scores were compared in the first and last test, a significant difference was observed between the post-treatment groups in the direction of the mobilization group. When the Visual Analogue Scale Resting Score, Quick-DASH and Shoulder Pain and Disability Index were compared before and after treatment, there was a significant decrease within the groups, but no significant results were found between the groups. As a result of the study, it was found that the first mobilization applied in addition to the conservative treatment program in individuals with subacromial impingement syndrome increased the flexion, abduction and external rotation angles and reduced night and activity pain. We recommend that it be included in the program of manual therapy applications added to the exercise therapy, which is considered the gold standard in the treatment of subacromial impingement syndrome, to increase the range of motion and reduce night and activity pain.
