İstanbul’daki hastane mutfaklarından alınan yüzey örneklerinde hijyenik durumun ve çalışan personelde hijyen farkındalığının belirlenmesi

Abstract

Amaç: Bu çalışma İstanbul'da bulunan üç özel hastanenin HACCP kalite güvence sistemi sertifikasına sahip mutfaklarından alınan yüzey örneklerinde hijyenik durumun ve çalışan personelde hijyen farkındalığının belirlenmesi amacı ile yapılmıştır. Yöntem: Çalışmada toplam 200 mutfak personeline yüz yüze sorma yöntemi ile demografik verilerin alınması ve hijyen bilgi düzeyini belirlemeye yönelik hazırlanmış 20 soruluk anket uygulanmıştır. Verilerin istatistiki değerlendirilmesinde IBM SPSS Statistics 22 paket programı kullanılmıştır. Anlamlılık p<0,05 düzeyinde değerlendirilmiştir. Ayrıca hastane mutfaklarında çalışan personelin el ve kıyafetlerinden, mutfak alet ve ekipman yüzeylerinden alınan swap örneklerinde Plate Count Agar, Violet Red Bile Agar, Baird Parker Agar, Yeast Extract Glucose Chloramphenicol Agar besiyerleri kullanılarak sırasıyla toplam aerobik mezofilik canlı bakteri, koliform ve fekal koliform bakteri, Staphylococcus aureus ve küf-maya sayımı yapılmıştır.Bulgular: Araştırmada personelin %26'sının eğitim durumunun ilkokul, %33,5'inin ortaokul ve %40,5'inin ise lise ve dengi okul olduğu ve %76,5'inin hijyen eğitimi aldığı tespit edilmiştir. Ankette sorulan sorulara verilen cevaplar göz önüne alındığında hijyen eğitimi alan personelin eğitim almayan personele oranla daha bilgili olduğu, ancak hijyen eğitimi alan personelin de yeterli bilgi düzeyine sahip olmadığı belirlenmiştir. Hastane mutfaklarında çalışan personelden alınan el örneklerinde toplam aerobik mezofilik canlı bakteri, koliform ve S. aureus ortalama değerleri sırasıyla 4,35±0,51 log kob/cm², 1,08±0,41 log kob/cm² ve 0,34±0,08 log kob/cm² düzeyinde belirlenmiş, fekal koliform grubu bakteriye rastlanmamıştır. Çalışmada soğuk üretim tezgahı, fekal koliform bakteri yükünün (0,34±0,39 log kob/cm²); personel eli, toplam aerobik mezofilik canlı bakteri (4,35±0,51 log kob/cm²), S. aureus (0,34±0,08 log kob/cm²) ve koliform bakteri yükünün (1,08±0,41 log kob/cm²); personel önlüğü ise küf/maya yükünün (0,29±0,13 log kob/cm²) en yoğun olduğu yüzeyler olarak belirlenmiştir.Sonuç: Çalışma kapsamındaki mutfaklarda HACCP sistemi uygulanmasına rağmen özellikle personel hijyeninde halen bir takım eksikliklerin bulunduğu, bunun en büyük sebebinin ise personel tarafından hijyen kurallarının tam olarak benimsenmemesi olduğu düşünülmüştür.Objective: This study was carried out with the aim of determining the hygienic conditions in the surface samples and also hygiene awareness of the working personnel in the 3 private hospital kitchen in Istanbul, which have HACCP quality assurance system certificate.Methods: In the study, a questionnaire consisting of 20 questions was prepared to determine the level of hygiene information and to get demographic data by using face to face method to 200 kitchen staff. Statistical evaluation of the data was made by using the IBM SPSS Statistics 22 package program. Significance level was assessed at p<0.05. In addition, total aerobic mesophilic bacteria, coliforms and fecal coliform bacteria Staphylococus aureus and mold-yeast counts were determined by using Plate Count Agar, Violet Red Bile Agar, Baird Parker Agar and Yeast Extract Glucose Chloramphenicol Agar media in the swap samples taken from the hands and clothes of the staff working in the hospital kitchens and also kitchen equipment surfaces.Results: In the survey, it was found that 26% of the staff educated in primary school, 33.5% in middle school and 40.5% in high school, also 76.5% of the staff trained about hygiene. When the answers to the questions asked in the questionnaire are taken intoconsideration, it was determined that hygiene trainingstaff were more knowledgeable than non-trainingstaff, but that hygiene training personnel did not havesufficient knowledge. While the average values of totalaerobic mesophilic bacteria, coliform and S. aureuswere determined as 4.35±0.51 log cfu/cm², 1.08±0.41log cfu/cm² and 0.34±0.08 log cfu/cm², respectively,in the samples taken from staff’s hands, the faecalcoliform bacteria was not found. In the study, thehighest fecal coliform bacteria load (0.34±0.39 log cfu/cm²) was found in cold production bench; the highesttotal aerobic mesophilic bacteria load (4.35±0.51log cfu/cm²), S. aureus (0.34±0.08 log cfu/cm²) andcoliform bacteria load (1.08±0.41 log cfu/cm²) wasfound in staff’s hands and the highest mold/yeast load(0.29±0.13 log cfu/cm²) was in staff uniform.Conclusion: Despite the implementation of theHACCP system in the kitchens of the study, it wasthought that there were still some deficiencies inthe personnel hygiene especially, the main reason istold that the hygiene rules were not fully adoptedby the staff.
