Vulvar crohn hastalığı: İki olgu sunumu ve literatürün gözden geçirilmesi

Abstract

Crohn hastalığı (CH), genellikle barsak dışı bulguları olabilen ve gastrointestinal sistemin herhangi bir bölümünü etkileyen, kronik seyirli inflamatuar granülomatöz bir hastalıktır. CH’nin eritema nodozum ve piyoderma gangrenozum gibi barsak dışı tutulumları iyi bilinmektedir. CH; fistül veya apselerle birlikte barsak inflamasyonunun semptom ve bulgularını verebileceği gibi, etkilenen barsak bölgesinden tamamen bağımsız bir alanda da semptom ve bulgular verebilir. Son bahsedilen form “Metastatik CH” olarak adlandırılmaktadır. Vulvar CH’nin tanısı, özellikle klasik barsak semptom ve bulgularının olmadığı durumlarda oldukça zordur. Bu çalışmada, genital şişlik ve akıntı şikâyetleriyle gelen, inflamatuar barsak hastalığı öyküsü ve barsak şikâyeti olmayan, iki olgu sunulmuştur.Crohn's disease (CD) is a chronic inflammatory granulomatous disorder that may affect any part of the gastrointestinal tract and is often associated with extra intestinal manifestations. Extraintestinal manifestations of Crohn's disease such as erythema nodosum and pyoderma gangrenosum are well recognized. It can also present with signs and symptoms of bowel inflammation with or without the presence of fistulas/abscesses, or as a distant lesion independent from the affected bowel. The latter form is named as "Metastatic CD". The diagnosis of vulvar CD is challenging, especially in the absence of classical intestinal symptoms. Here we report two cases who presented with genital swelling and discharge and without history of inflammatory intestinal disease and intestinal complaints.
