Periferik vestibüler hipofonksiyon tanılı hastalarda sanal gerçeklik tabanlı dikkat, bakış ve postür egzersizleriyle vestibüler rehabilitasyon

Abstract

Bu çalışmada periferik vestibüler hipofonksiyon (PVH) tanısı almış 25 kişide Sony Playstation 4 Sanal Gerçeklik (SG) tabanlı vestibüler rehabilitasyon (VR) uygulanmıştır. Hastalar haftada 2 kez olmak üzere 4 hafta boyunca toplamda 8 seans VR eğitimi almıştır. Hastalara rehabilitasyon öncesi (RÖ), rehabilitasyon sonrası (RS) ve RS 2. ay takip dönemlerinde Baş Dönmesi Engellilik Envanteri (BEE) ve Bilgisayarlı Dinamik Postürografi'den (BDP) 6 alt test grubu uygulanmıştır. Ek olarak, hastalara her hafta tamamlanan seans sonunda 4 kez Cybersickness (Siber Duyarlılık) Anketi ile SG sonrası baş ağrısı, bulanık görme, terleme, göz açık-kapalı iken dengesizlik, bulantı, kusma hissi semptomlarının gelişip gelişmediği sorgulandı. Hastaların RÖ BEE puanları ile RS ve RS 2. ay takip dönemlerinde puanları arasında (p<0,00) istatistiksel olarak anlamlı farklılık elde edildi. Hastaların RÖ BEE puanları her dönem azalma göstermiştir. Bununla birlikte hastaların Duyusal Organizasyon Testi'ndeki (DOT) kompozit puanları arasında RÖ, RS ve RS 2. ay arasına göre kıyaslandığında istatistiksel olarak anlamlı fark elde edildiği saptandı. PVH tanılı hastalarımızın RS dönemindeki Adaptasyon Testi, Denge Sınırları Testi, Ritmik Ağırlık Aktarma Testi ve Tek Ayak Üzerinde Duruş Testindeki kompozit değerleri RÖ testindeki değerlere göre anlamlı derecede iyileşme gösterdi. Sadece, Çömelerek Ağırlık Aktarma Testi'ndeki değerlerde dönemler arasında istatistiksel olarak anlamlı bir fark elde edilmedi. Çalışmamızın sonucunda SG tabanlı VR protokolümüzün PVH tanısı almış hastalarda rehabilitasyonun önemi ortaya konarken orta dönemde de rehabilitasyonun etkinliğini sürdürdüğünü gösterdi.In this study VR-based VRT program which utilized, Sony Playstation 4 was applied to 25 patients (between age of 18 to 60) were diagnosed with peripheral vestibular hypofunction (PVH). VR-based VRT program was applied to the patients, 2 sessions per week, for 4 weeks, in total 8 sessions. All patients underwent the Dizziness Handicap Inventory (DHI) and Computerized Dynamic Posturography (CDP) at before, just after and 2nd month follow-up of the VRT program. In addition, Cybersickness Questionnaire was applied to the patients at the end of the VR based VRT session, which was completed every week. In addition, at the end of the session completed every week, the patients were questioned whether the symptoms of headache, blurred vision, sweating, imbalance when the eyes were open and closed, nausea and vomiting developed after SG using the Cybersickness Questionnaire. There was a statistically significant difference in pre-VR DHI scores between both post-VR and second-month post-VR follow-up periods (p <0.000). Pre-VR DHI scores of the patients gradually decreased in each period. However, a statistically significant difference was found between the composite scores of the patients in the Sensory Organization Test (SOT) compared to the pre, post and the 2nd month post VR. Composite values in Adaptation Test, Limits of Stability, Rhythmic Weight Shift Test, and Unilateral Stance Test showed significant improvement compared to values in the pre-rehabilitation test in patients diagnosed with PVH (p<0,000). As a result of our study, VR-based VRT protocol demonstrates the importance of rehabilitation for patients with PVH, and that rehabilitation continues to be effective also in the mid term.
