Zihinsel engelli bireylerde dual task denge eğitiminin bilişsel fonksiyonlar üzerine etkisi

Abstract

Çalışmalar beyin faaliyetlerimizin çok farklı şekillerle ve yollarla meydana geldiğini, düşünmenin, izlemenin, hareketin aktif yapılmasının veya pasif gerçekleştirilmesinin aynı beyin bölgelerini benzer şekilde aktive ettiğini göstermektedir. Bu çalışma kapsamında zihinsel engelli bireylerin zihinsel performanslarını geliştirebilmek adına, son zamanlarda sık kullanılan kognitif rehabilitasyon yöntemlerinden biri olan dual task çalışmaları kullanılmıştır. Ayrıca zihinsel engelli bireylerin eğitimlerine fizyoterapi programı eklenerek, motor becerilerinin geliştirilmesinin zihinsel performansları açısından etkili ve gerekli olup olmadığının belirlenmesi amaçlanmıştır. Çalışmaya 6-13 yaşları arasında, zihinsel engellilik düzeyleri IQ=50-79 olan 45 zihinsel engelli birey dahil edilmiştir. Bireylerin engel durumları sağlık raporuyla, bilişsel durumları da Milli Eğitim Müdürlüklerine bağlı Rehberlik Araştırma merkezlerince yapılan değerlendirme sonucu verilen raporla belirlenmiştir. Katılımcılar 15'şer kişilik 3 gruba rastgele ayrılmıştır. DTDE grubuna bilişsel eğitim ve dual task denge eğitimi; SDE grubuna standart denge eğitimi ve bilişsel eğitim, ZE grubuna ise sadece bilişsel eğitim uygulanmıştır. MOXO Dikkat Testi, Wisc-R Zeka Testi, Bruininks-Oseretsky Motor Yeterlilik Testi, COGNİBOARD ve Fonksiyonel Uzanma kullanılarak çalışma öncesi, sonrası ve üç ay sonrasındaki sonuçlar değerlendirilmiştir. Çalışmamızdan elde ettiğimiz bulgulara göre; DTDE grubundaki katılımcıların uygulama sonrası Wisc-r zeka testi, MOXO Dikkat Testi ve Bruininks-Oseretsky Motor Yeterlilik Testinde alınan puanlar uygulama sonrasında uygulama öncesine göre anlamlı olarak daha yüksek bulunmuştur. Fonksiyonel Uzanma Testinde ise DTDE grubunun hem uygulama sonrası hem de uygulamadan 3 ay sonra aldıkları puanların, kontrol gruplarına göre daha yüksek olduğu görülmüştür. Zihinsel engelli bireylere uygulanacak özelleşmiş bir fizyoterapi programının bu bireylerin zihinsel performansları üzerine etkisini gösterdiği görülmüştür. Dolayısıyla çalışmamız bu yönüyle literatüre katkı sağlayacak niteliktedir.Studies show that our brain activities occur in many different ways and ways, that thinking, watching, activating or passive activating the same brain regions alike. Within the scope of this study, dual task studies, which is one of the most frequently used cognitive rehabilitation methods, were used in order to improve the mental performance of individuals with intellectual disabilities. In addition, it is aimed to determine whether the development of motor skills is effective and necessary in terms of mental performance by adding a physiotherapy program to the education of mentally disabled individuals. Forty-five mentally handicapped individuals, aged 6-13, with IQ=50-79, were included in the study. The disability status of individuals was determined by the health report, and their cognitive status was determined by the report given as a result of the evaluation made by the Guidance Research Centers affiliated to the National Education Directorates. Participants were randomly divided into 3 groups of 15 each. Cognitive training and dual task balance training for the subject group; Standard balance training and cognitive training were applied to the 1st control group, and only cognitive training to the 2nd control group. Results before, after and three months were evaluated using the MOXO Attention Test, Wisc-R Intelligence Test, Bruininks-Oseretsky Motor Proficiency Test, COGNIBOARD, and Functional Reach. According to the findings of our study; The post-application Wisc-r intelligence test, MOXO Attention Test and Bruininks-Oseretsky Motor Proficiency Test scores of the participants in the experimental group were found to be significantly higher after the application than before the application. In the Functional Reach Test, it was observed that the scores of the experimental group both after the application and 3 months after the application were higher than the control groups. It has been observed that a specialized physiotherapy program to be applied to individuals with intellectual disabilities has an effect on the mental performance of these individuals. Therefore, our study will contribute to the literature in this aspect.
