Pontoserebellar köşe tümörü cerrahisi sonrası dengenin değerlendirilmesi

Abstract

Amaç: Pontoserebellar Köşede meydana gelen tümörler Pontoserebellar Köşe (PSK) tümörü olarak adlandırılır. Bu çalışma PSK tümörü cerrahisi geçiren hastaların dengelerini değerlendirmek amacı ile yapılmıştır. Yöntem: Beyin Sinir ve Omurilik Cerrahisi Bölümünde takip edilen, 2011-2015 yılları arasında PSK tümörü nedeni ile operasyon geçirmiş 30 hasta çalışmaya dâhil edildi. Hastaların operasyon sonrası geçen süre ortalaması 22.67 (1-54 ay) aydır. Çalışmaya katılan hastaların demografik bilgileri alındı. Değerlendirmelerde alt ekstremite kas gücü için manuel kas testi; denge için Romberg test, Tek ayak üstünde durma, Düz beş adım yürüme; yaşam kalitesi Short Form-36 (SF-36) ve baş dönmesi Baş Dönmesi Engellilik Envanteri ile değerlendirildi. Bulgular: Çalışmanın sonunda özellikle Tek ayak üstünde durma testleri olmak üzere denge parametrelerinin normal kişilere göre düşük değerde olduğu gözlemlendi. Hastalarda alt ekstremite kas zayıflığı görülmedi. Baş Dönmesi Engellilik Envanteri’ne göre hafif derecede engellilik görüldü. SF-36’ya göre yaşam kalitelerinde ortalama değerlere yakın skorlar elde edildi. Sonuç: Çalışmaya göre bu hastaların operasyon öncesi ve sonrası denge değerlendirmelerinin ihmal edilmemesi gerektiği ve erken dönem rehabilitasyona başlamalarının gerekli olduğu düşünülmektedir.Purpose: Tumors from the Cerebellopontin angle are called as Cerebellopontin angle (CPA) tumors. This study was conducted with the aim of evaluating the balances of patients who underwent CPA tumor surgery. Methods: Thirty patients who were followed in the Department of Neurology and Spine Surgery and had undergone CPA tumorinduced surgery between the years 2011 and 2015 were included in the study. Post operative average duration of patients was 22.67 months (1-54 months). Demographic features of the patients participating in the study were obtained. For evaluation; manual muscle testing for lower limb muscle strength; Romberg test for equilibrium, Standing on one foot, Walking five steps straight were performed; Quality of life was assessed by Short Form-36 (SF-36) and Dizziness Handicap Inventory (DHI) was used. Results: At the end of the study, it is observed that the equilibrium parameters were lower than normal persons, especially for the ones standing test on one foot. No lower limb muscle weakness was seen in the cases. A slight degree of disability was seen in DHI results. According to SF-36, scores that close to the average values in quality of life were obtained. Conclusion: According to the study, it is thought that the balance evaluations of these patients before and after operation should not be neglected and it is necessary to start early rehabilitation.
