Bilgisayar destekli tasarım ve üretim (CAD/CAM) teknolojisi ile üretilen korseleri kullanan adolesan idiopatik skolyozlu bireylerde postüral denge ve plantar basıncın incelenmesi

Abstract

Omurganın 3 boyutlu deformitesi olan skolyoz, gövdede asimetri oluşturması nedeniyle alt ekstremitede yüklenme değerlerini etkiler. Bu durum yüklenme asimetrileri ve postüral dengesizliğe neden olabilmektedir. Adolesan idiopatik skolyozlu (AİS) bireylerde Bilgisayar Destekli Tasarım ve Bilgisayar Destekli Üretim (CAD-CAM) teknolojisi ile üretilen korse kullanımında oluşan plantar basınç ve denge parametreleri değişikliklerini incelemek amacıyla yaptığımız çalışmamıza; Bilim Ortez Protez Merkezi'ne korse uygulanan 10-19 yaş arasında 30 AİS'li birey dahil edildi. Bireylere; deformitelerini nasıl algılarını tespit etmek üzere Walter Reed Görsel Değerlendirme Skalası (WRVAS) ve kozmetik defekti ölçmek amacıyla Posterior Gövde Simetri İndeksi (POTSİ) değerlendirmesi yapıldı. Bunlara ilave olarak korsesiz ve CAD-CAM yöntemi ile üretilen korseli pedobarografik analiz yapıldı. Pedobarografik analizlerde; statik basınç, stabilometrik-salınım ve dinamik basınç değerleri elde edildi. Verilerin analizinde veriler "Statistical Package for Social Sciences" IBM SPSS Version 22.0 kullanıldı. Elde edilen verilere göre; korse kullanımının statik analiz sonuçları incelendiğinde yüklenme değerlerinde istatistiksel olarak anlamlı bir fark saptanmadı (p>0,05). Dinamik analiz verilerinde ise korse kullanımının sol adım uzunluğu ve ivmeyi azalttığı belirlendi (p<0,05). Denge ve salınımın değerlendirildiği stabilometrik analizde ise korse kullanımında ve korsesiz durum arasında anlamlı bir fark tespit edilmedi (p>0,05). Bu çalışma "2019-22" proje numarası ile İstanbul Medipol Üniversitesi Bilimsel Araştırma Projeleri (BAP) desteği ile gerçekleştirildi.Scoliosis is a 3-dimensional deformity of the spine, as it creates asymmetry in the trunk, it affects the loading values in the lower limbs. This may cause loading asymmetries and postural imbalance. In our study we aimed to examine the changes of plantar pressure and balance parameters in the use of brace produced by Computer Aided Design and Computer Aided Manufacturing (CAD-CAM) technology in patients with adolescent idiopathic scoliosis (AIS). The study included 30 participants with AIS between the ages 10-19 who were applied for a brace tretment at the Bilim Orthotics and Prosthetics Center. Walter Reed Visual Evaluation Scale (WRVAS) used to detect deformity perceptions, and posterior body symmetry index (POTSI) used to measure cosmetic defects in individuals with AIS. Additionaly, Pedobarographic analysis was performed with braces produced by CAD-CAM method. Static pressure, dynamic pressure, and stabilometric-oscillation values were obtained in pedobarographic analysis. "Statistical Package for Social Sciences" IBM SPSS Version 22.0 was used to perform the analysis of the data. According to the data obtained; it was found that the using of braces did not change the loading values to be statistically significant in static analysis (p> 0.05). According to dynamic analysis results, it was determined that the using of a brace significantly reduced length of steps and acceleration on the left side (p <0.05). In the stabilometric analysis in which balance and oscillation were evaluated, no significant difference was found between the using of braces and without brace condition (p> 0.05).
