Sağlık hizmetlerinde yapay zeka uygulamaları: Yoğun bakım örneği

Abstract

Sepsis, vücutta gelişen enfeksiyona neden olan bakterilerin kana karışması ile bağışıklık sisteminin bu bakterilere karşı büyük bir savunma göstermesi; bu nedenle vücutta organ yetmezliğinin gerçekleşmesi olarak bilinmektedir. Sepsis enfeksiyonunun erken teşhisi yapılamaz ve hızla tedaviye başlanamaz ise çoklu organ yetmezliği sonucu ölüme neden olabilmektedir. Sepsis tanısının konulması amacıyla Ardışık Organ Yetmezliği Değerlendirme (Sequential Organ Failure Assessment Score: SOFA) ve Hızlı Ardışık Organ Yetmezliği Değerlendirme (quick Sequential Organ Failure Assesment: qSOFA) skorlama sistemleri kullanılmaktadır. SOFA skorlamasının zaman alıcı olması, qSOFA'nın ise düşük performans göstermesi nedeniyle bu denli kritik bir hastalığın erken teşhisi için daha doğru klinik tahmin modellerine gereksinim duyulmaktadır. Bu tez çalışması kapsamında sepsis enfeksiyonunun gelişimini erken tahmin etmeye yönelik bir yapay zeka çalışması gerçekleştirilmiştir. Çalışmada, Beth Israel Deaconess Tıp Merkezi'nin yoğun bakım ünitelerinde kalan hastaların klinik verilerinden oluşan MIMIC-III veri tabanı kullanılmıştır. Öncelikle veriler üzerinde denetimsiz makine öğrenmesi gerçekleştirilmiştir. FCM algoritması kullanılarak gerçekleştirilen öğrenme sonunda, küme geçerlilik indeksleri dikkate alınarak optimum kümeler elde edilmiştir. Küme temsilcileri PCA yöntemi ile iki boyuta indirgenerek hekimlerin sepsis örüntüsünü takip edebileceği bir model geliştirilmiştir. Derin öğrenme algoritması LSTM kullanılarak hastanın sonraki saat içinde sepsis olup olmayacağı %82 başarı ile tahmin edilmiştir. Araştırmada gerçekleştirilen yapay zeka modelinin, kritik hastalıkların öngörülerek hekimlerin karar verme sürecine yardımcı olabileceği, sağlık merkezinin rekabet avantajı sağlayan dinamik yeteneklerini geliştirebileceği ve tüm bunlar sayesinde, sağlıkta inovatif stratejilerin geliştirilmesinde rol oynayabileceği vurgulanmıştır.Sepsis infection, which is one of the most important causes of death in intensive care services, is one of as a serious global health crisis. Sepsis infection can cause death because of multiple organ failure. Early diagnosis and rapid treatment process are crucial. Sepsis can be diagnosed along with clinical findings, laboratory results, SOFA and qSOFA scoring systems. Sophistication and time consuming of the SOFA scoring system and poor performance of the qSOFA scoring system are the main disadvantages for sepsis where early diagnosis is essential. Therefore, more accurate clinical prediction models are needed. Within the scope of the thesis, an artificial intelligence study was employed to predict sepsis infection earlier. The MIMIC-III database, which consists of detailed clinical data of patients treated in the intensive care units of Beth Israel Deaconess Medical Center, was used in the study. In the beginning of the study, unsupervised machine learning was performed using the data obtained from the patients using the FCM algorithm. Optimum clusters were obtained by considering the cluster validity indices. Cluster representatives were reduced to two dimensions with Principal Component Analysis method and a model was developed in which doctors could follow the sepsis pattern. It is predicted 82% accuracy whether the patient would have sepsis within the next hour by using the deep learning algorithm LSTM. This research emphasizes the artificial intelligence model implemented and critical diseases can be predicted, and doctors can help the decision-making process. Therefore, health centers can improve their dynamic capabilities which provide competitive advantages. In the lights of all these advantages, artificial intelligence can play a essential role in the development of innovative strategies in health centers.
